SUMMARY

The Lead Care Manager (LCM) position provides family-centered, member-focused, relationship-based care coordination services to children and youth enrolled in Enhanced Care Management services. The LCM is responsible for the wholistic coordination of all internal and external care that assigned members receive and ensures the timely assessment of comprehensive needs and linkage to and utilization of community resources to meet those needs. This position must effectively utilize broad knowledge of service and benefits systems, community resources, and Agency policies and procedures to make quick, appropriate independent judgments in the course of daily work.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Supports and promotes the mission of the Agency: Foothill Family empowers children and families on their journey to achieve personal success.
  • Coordinates all service activities of the members' service providers and support networks to ensure a seamless experience for the members and the non-duplication of services.
  • Leads outreach and engagement of ECM-eligible members resulting in successful enrollment into the program.
  • Oversees the provision of ECM services and implementation of the care plan.
  • Offers services where the member lives, seeks care, or finds most easily accessible and within managed care plan guidelines.
  • Connects ECM members to social services and support they may need, including driving members to appointments or arranging transportation.
  • Advocates on behalf of ECM members with health care professionals and other service and/or resource systems.
  • Utilizes motivational interviewing, trauma informed care, and harm reduction approaches.
  • Coordinates with hospital staff on ECM member discharge plans.
  • Drives or accompanies ECM members to office visits and according to managed care plan guidelines.
  • Monitors treatment adherence, including medication.
  • Provides health promotion and self-management training.
  • Carries a full caseload of 50 ECM members.
  • Identifies gaps in and barriers to service, documenting in the member's care plan, and communicating these to the multidisciplinary care team to develop and implement a response.
  • Prepares for and participates in individual and group supervision meetings.
  • Behaves in an ethical, professional manner in support of Agency culture.
  • Identifies and raises for discussion, with management staff, potential workplace and morale issues, making recommendations for addressing them.
  • Displays positive and attentive attitude at site meetings, as well as department, program, and Agency staff meetings.
  • Works effectively with the program and site coordinators and the department head, accepting direction and implementing policies and procedures reflective of this direction.
  • Travels between Foothill Family sites for training, meetings, and other in-person interactions.
  • Attends in-person meetings and events at various locations within the Los Angeles County and surrounding areas.
  • Displays sensitivity to the service population's cultural and socioeconomics characteristics.
  • Performs work in a safe manner at all times and ensures that other individuals do not put themselves or others at risk.
  • Follows all policies, procedures and legal mandates including HIPAA, member confidentiality, releases of information, protected health information (PHI) and member files.
  • Ensures performance and behaviors support the Agency's performance quality improvement (PQI) goals and objectives.
  • Reports to work on time and maintains reliable and regular attendance.
  • Performs other duties as assigned.

POSITION REQUIREMENTS
* BSW or BA in a social science or related field, with 7 plus (10+ preferred) years Case Management experience required.
* Three to 5 years of experience conducting comprehensive needs assessments and developing corresponding service plans.
* Extensive case management and linkage experience addressing needs including but not limited to physical, behavioral, developmental, oral health, long-term services and supports, specialty mental health services, and community supports.
* Knowledge of rights and available resources for Medi-Cal Beneficiaries.
* Experience coordinating care and collaborating with various county departments including Department of Mental Health, Department of Public Health, Department of Children and Family Services, and Department of Social Services.
* Experience conducting Motivational Interviewing and providing trauma informed care. Knowledge of harm reduction strategies is a plus.
* Excellent verbal and written communication skills; ability to conceptualize.
* Ability to comply with all Medi-Cal quality assurance requirements including timely and accurate documentation of member information and service activities in addition to upkeep of member records.
* Possess organizational skills to manage high activity level, provide needed services and attention to each member while maintaining all reporting and documentation requirements in a timely and accurate manner.
* Excellent people and problem-solving skills; ability to work independently as well as a member of a team.
* Ability to work with staff and members from diverse ethnic, cultural, and economic backgrounds, and to provide services in a non-judgmental manner.
* Affinity for working with adolescents and young children.
* Demonstrates initiative, flexibility, creativity, and enthusiasm.
* Ability to develop routine reports and correspondence.
* Valid CA Driver's License and maintains insurability on the Agency's auto liability policy (including a minimum of 2 years driving experience after receiving license) and maintains the California state required auto insurance liability limits.
* Bilingual English/Spanish skills required.
