Ambulatory Clinical Pharmacy Job Summary

Under minimal supervision, the clinical specialist pharmacist is responsible for reviewing and approving medications, providing drug therapy management services for patients in assigned area, and provision of pharmacy services, including drug information, patient education, and patient monitoring. Initiate, adjust, and monitor medications pursuant to provider order and consistent with department policy, and rules and regulations promulgated by state and federal laws and The Joint Commission. The clinical specialist pharmacist is the subject matter expert in a specialized pharmacy area and collaborates with pharmacy leadership in development of policies and procedures, clinical care guidelines, and other work required to effectively provide high quality pharmaceutical care.

Essential Functions:
  • Patient Care Activities and Daily Operations:
    * Interprets and clarifies orders for all types of medications.
    * Researches and responds promptly to drug information requests.
    * In collaboration with providers, monitors drug therapies for appropriateness and formulary adherence, ensuring drug regimen supports financial stewardship.
    * Designs therapeutic drug regimens or plans based on patient-specific information that includes establishing a therapeutic endpoint, selecting appropriate drug and related therapy, and establishing an appropriate dosing regimen.
    * Monitors drug therapy for potential drug-drug, drug-food, and drug-disease interactions.
    * Advises patients, family and medical staff on use of various medications including dosage, side effects, drug interactions and composition.
    * Directly supervises the work of pharmacy technicians and support staff as required by the Colorado Board of Pharmacy
    * Prepares or oversees the compounding, labeling and packaging of sterile or non-sterile medications, products and pharmaceuticals in accordance with all regulatory requirements as applicable.
    * Maintains secure storage and records of all medications in accordance with manufacturer, state and federal requirements.

  • Training and Development of Team Members:
    * Acts as a consultant on medication related issues and assisting in coordinating the drug utilization programs.
    * Assists in the development and maintenance of policies & procedures when warranted.
    * Complies with established departmental and DHHA policies and procedures, quality improvement, and safety initiatives.
    * Completes and meets the competency standards including education and training that is specific to the patient populations and practice site served. Keeps job knowledge current.
    * Assists in the training of pharmacists, pharmacy residents, pharmacy interns, pharmacy students, and pharmacy technicians.
    * Attends and participates in departmental and/or system-wide meetings, in-services, huddles, and committees. Identifies clinical and/or operational opportunities for improvement, works to resolve problems in a manner that will improve patient safety outcomes and clinic efficiency.

  • Leads Quality Improvement Projects and Initiatives:
    * Identifies and implements recommendations for process improvement including areas of cost effectiveness and efficiency
    * Resolves identified medication and/or patient safety problems in a manner that will improve future practice
    * Understands patient safety goals, core measures, and other quality metrics that affect area of practice
    * Assists in development of programs to meet goals, core measures, and quality metrics
    * Collects and analyzes data to monitor effectiveness of improvement work

  • Professional Development:
    * Completes and meets all competency standards including education and training specific to practice area
    * Attends and participates in assigned committees, departmental meetings, and training as assigned
    * Identifies opportunities for learning and training to advance own professional competence.

Education:
  • Doctorate Degree PharmD (preferred) Required or
  • Bachelor's Degree BPharm Required
  • Colorado Pharmacist License Required

Work Experience:
  • 4-6 years PGY2 residency or 4 years minimum experience in area of specialty Required

Licenses:
  • PHA-Licensed Pharmacist - BPS - Board of Pharmacy Specialties Required
  • BCPS-Board Certified Pharmacotherapy Specialist - BPS - Board of Pharmacy Specialties
  • ACLS-Advanced Cardiac Life Support - AHA - American Heart Association
  • PALS-Pediatric Advanced Life Support - AHA - American Heart Association
  • BLS-Basic Life Support (BLS/CPR) - AHA - American Heart Association
  • PBID-Pharmacy Based Immunization Delivery - BPS - Board of Pharmacy Specialties

Knowledge, Skills and Abilities:

  • Ability to work as a team member.
  • Effective oral and written communication skills.
  • Strong organizational skills and attention to detail.
  • Strong analytical skills necessary to identify and resolve problems related to drug therapy.
  • Ability to instruct, direct, and evaluate employees- will be required to assume responsibility for daily operations.
  • Ability to work independently and with little supervision.
  • Prior experience and/or knowledge of process improvement practices
  • Thorough knowledge of the practices and principals involved in the preparation of nonhazardous and hazardous sterile compounds.
  • Proficiency with Epic, preferred
  • Proficiency with MS Office, preferred.
  • Proficiency with Pyxis, preferred.
