CLINICAL MANAGER

The Manager will trackand monitorallpsychiatrydischargesfor77bedsacrossMMCand Mt Vernon. The Coordinator will be responsibleforthe timely communicationand coordinationacrossMoses, Wakefield, Weilerand Mt VernonPsychiatryteamsacrossthe inpatientand ED settings. The goal is to facilitate a seamless transitionforhigh acuitypsychiatrypatients discharged from the ED, medical,and surgical settings, ensuring a smooth hand-offand placement to utilize full system acutepsychiatrycapacity. Additionally, the coordinator is expected to offer case management supportforhighly complex cases in our inpatientpsychiatryand medical settings.

Timely communicationand coordinationacrossMoses, Wakefield, Weilerand Mt VernonPsychiatryteamsacrossthe inpatientand ED settings.

Evaluatingand assessing psychiatric admission needs from ED, medicaland surgical settings

Ensuring timely communicationand placement of patients post-discharge

Provide case management supportforcomplex cases

Attend complex care meetings at each campus

Coordinate communicationacrossthe entire system concerning the availability of psychiatricbeds, patients awaiting bed placement,and those awaiting discharge.

Leading cross-campus huddles multiple times per week to discuss key data pointsand patient needsacrosscampusesand providing directionand decision-making on key placement issues

Requirements

Education BSN or LCSW

NYS licenseand Registration

Five years experience in a clinical setting

Strong leadership skills required

Knowledge of EPIC, Microsoft,and Outlook
