We are seeking a Clinical Social Worker who will be responsible for providing comprehensive case management to identified clients and their families. The Social Worker works closely with families and Early Learning staff to stabilize them and connect them to longer term supports.

Duties & Responsibilities:



  • Partner with identified families experiencing homelessness, domestic violence, severe mental illness, or other crisis to connect them to resources. Provide on-going clinical support as needed and maintain regular communication until family stabilizes.
  • Make referrals for Early Learning, Family Child Care and School Age families whose members need outpatient therapy and CBHI services. Maintain contact with the referral sources in order to support the family's connection to them.
  • Coordinate with team members across programs and divisions to provide wrap around care for the family. Organize and facilitate focused case management as needed.
  • Coordinate and communicate with external agencies and supports to ensure follow up and positive support for individual families.
  • Provide information and support related to Early Learning staff regarding mental health, substance abuse or other issues as needed.
  • Work with Parent Engagement and BHC staff to develop and co-facilitate evidence-based groups such as Nurturing Families, Circle of Security Parent groups, or other options.
  • Develop and/or facilitate reflective and/or communities of practice for educators, staff or families as needed.
  • Participate in Behavioral Health Center meetings and clinical supervision.
  • Attend case management meetings for individual families as needed.
  • Offer individual consultation to teachers, family service workers, inclusion specialists and coaches as needed.
  • Complete clinical documentation in appropriate software. Manage confidential information within protocol.
  • Maintain accurate case records by documenting all services rendered including up to date case notes, telephone contact logs and referrals. Enter required data into the agency databases with reporting as required.
  • Provide emergency response management to address any crises or safety issues that may arise.
  • Perform other duties as assigned.


Qualifications:



  • Experience with children and families in a clinical social work capacity.
  • Attained licensure as an LCSW, LICSW, or LCMH.
  • Master's degree in social work or counseling.
  • Must be able to pass EEC Background check which includes fingerprinting
  • Must be able to show proof of MMR and recent physical
  • Desire to work with low income, culturally diverse, and at-risk families and individuals.
  • Excellent customer service, organizational, oral and written communication skills.
  • Ability to manage multiple priorities and work in a fast-paced environment.
  • Proficient computer skills required in basic computer software and data collection systems.
  • Excellent crisis management skills.
  • Ability to maintain accurate records and confidentiality of information.
  • Bilingual (English/Spanish, English/Khmer, English/Portuguese) preferred
