Summary

Using personal drive and collaborative energy, develops relationships with organizations and individuals that may refer patients to Schwab Rehabilitation Hospital and Holy Cross Rehabilitation Unit. Using strong multi-tasking skills to effectively provides on-site assessments at referring hospitals to determine patient appropriateness for rehabilitation. Work with referral sources and Schwab's Admitting Department to facilitate timely admissions. Assist Schwab in achieving budgeted inpatient census.

Essential Job Responsibilities



  • Through face-to-face meetings make contacts and develop strong relationships with organizations and individuals that may refer patients to Schwab and Holy Cross Rehabilitation. These include discharge planners, social workers, case managers, nurses, and physicians.
  • Education of clients, family members, and referral sources on acute and sub-acute rehabilitation advantages, differences, and associated criteria for the effective and efficient utilization of program services and available resources.
  • Works effectively with appropriate hospital staff members to provide services to existing referral sources and constantly strives to increase number of referrals.
  • Proactively seeks to identify new referral sources by making an identified number of quality appointments per day and contacts per week.
  • Demonstrates strong ability to multi-task in conducting analysis, decision-making, and time management when reviewing referrals for admission.
  • Assist with scheduling informational presentations for referring individuals and organizations.
  • Performs on-site and/or record review including telephonic pre-admission screening and clinical assessment and responds to patients, families and hospital referral source with a tentative 24-hour admission commitment. Responsive and attentive to needs of the referral source.
  • Assesses pre-morbid functional status, personal interests, cultural considerations and discharge plans affecting the rehabilitation process in a timely manner. Discusses assessment with physician to assist with medical determination.
  • Coordination of smooth transition from the referring facility to Schwab Rehabilitation Hospital or Holy Cross Rehabilitation unit. This includes ensuring patient is medically stable and updated medical records are provided during admission.
  • Provides recommendations and referral to appropriate level of care for candidates who are denied admission.
  • Completes all required pre-admission screening documentation on time and accurately to determine that services are appropriate, reasonable and necessary. Ensures physician has signed off before admission.
  • Collaborates with supervisor to identify and improve service to referral sources through creative and dynamic approaches.
  • Dynamic team member that works with other Outreach Coordinators to ensure volume goals are achieved at Schwab Rehabilitation Hospital and Holy Cross Rehabilitation Unit.
  • Demonstrates excellent assessment skills and understanding of the insurance and managed care qualifications for acute and sub-acute rehabilitation in the review, preparation, and coordination of referrals.

Customer Service Responsibilities

Demonstrate excellent customer service and standards of behaviors as well as encourages, coaches, and monitors the same in team members. This individual should consistently promote teamwork and direct communication with co-workers and deal discretely and sensitively with confidential information.

Contribute by identifying problems and seeking solutions. Promote patient/family satisfaction where possible; participates in departmental efforts to monitor and report customer service.

Essential Job Qualifications



  • Bachelor's Degree Required
  • A minimum of 2 years of experience in sales, marketing or rehabilitation required.
  • Current State of Illinois Drivers License, Car Insurance, Professional IL licensure preferred (Nursing, Therapies, Social Work, Case Management)

Compensation starting from $35.58 per hour
