$2,000 Sign On Bonus

Responsibilities:

  • Assess the physical and psychosocial status of patients in all settings by obtaining appropriate patient histories, administering physician/primary care provider orders, nursing assessment, medication administration reconciliation, take and monitor vitals of clients on the unit, medication administration and documentation.
  • Renders and supervises care of clients on the unit.
  • Monitors medication and consultation with prescribers.
  • Provide appropriate health care and coordinates referrals as prescribed by providers.
  • Participates in the evaluation of client's condition and progress with team members.
  • Promote education and adherence to prescribed treatment protocols.
  • Facilitates wellness and medically related psychoeducational groups to individuals.
  • Records all client information within electronic medical record system daily.
  • Assists RN on daily basis.
  • Offer nursing consult on emergency service evaluations.
  • Work with clients to coordinate care with their PCP, psychiatrist, and other professionals including but not limited to medication refills and f/u medical appointments.
  • Demonstrates ability to communicate effectively and professionally with clients, staff, and providers.
  • Demonstrates proficiency in completing electronic documentation in compliance with grant requirements, agency, state, and federal guidelines and policies.
  • Knowledge of medical, substance abuse and mental health nursing
  • Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients and visitors.

Qualifications:

  • Grade N04: Acute Care LPN: Graduate of an accredited school of Nursing with current license as a Licensed Practical Nurse in the state of Massachusetts.
  • Previous clinical experience working with substance abuse and mental health.

  • Demonstrated strong work ethic, ability to work independently, and ability to contribute to team objectives required.
  • All bilingual candidates are strongly encouraged to apply.
  • Current valid US-issued driver's license and ability to provide registered, inspected and insured automobile for work related purposes including transporting clients in your own vehicle
