Program Summary

The Family Practice & Counseling Network (FPCN) provides comprehensive primary care, dental and behavioral health services. The network's mission is to provide person-centered, integrated and comprehensive health services to individuals and families across their lifespan. The network promotes resiliency and well-being among patients, staff and surrounding communities.

We are a network of 3 comprehensive health centers, one convenient care center and six school-based health centers in Philadelphia.

Today, FPCN serves more than 15,000 patients annually and is a NCQA certified Patient-Centered Medical Home and recognized by HRSA as a National Quality Leader for Behavioral Health.

Position Summary

Upholds the mission of the organization by assuring the delivery of primary care that is evidenced-based, satisfying to patients, and cost-effective; promotes a fiscally sound organization.

Essential Duties and Functions

Quality of Care:

1. Supervise and support all of Primary Care to ensure high quality, integrated health care to all patients.

2. Provides evidence-based primary care directly to patients carrying out all responsibilities as outlined in the job description of CRNP, including adherence to Clinical Practice.

3. Ensures staff in the Primary Care Department receive best practice updates and trainings specific to their roles and skill levels.

4. Responds to new trends in healthcare and opportunities for service expansion as feasible and when benefitting the community's needs.

5. Manages Primary Care Population Health management

6. Provides direct patient care

Guidelines and Productivity Standards:

1. Responsible for hire, orientation, evaluation, and termination of all Nurse Practitioner and RN staff.

2. Oversight of Primary Care Departments office coverage to ensure good access to care

3. Ensures all Primary Care offices are meeting productivity expectations

4. Responsible for site level FTE balance and empanelment for Nurse Practitioners.

5. Develops, Monitors and Disseminates evidenced-based guidelines to all of Primary Care.

6. Supports FPCNs 340b Pharmacy program and liaisons with Contract Pharmacy to ensure optimal practices

7. Acts as liaison concerning OSHA, FTCA, and other governing regulations relative to Primary Care services.

8. Facilitates monthly NP meetings, Informatics meetings, PC Site Director meetings.

9. Provides oversight of medication supplies, inventory, and utilization.

Administrative:

1. Serves as a member of the Senior Leadership Team

2. Collaborates with Informatics Team on the optimal use of our EHR and ensures all Primary Care staff receive training in utilization

3. Ensures compliance with the Vaccines for Children program including training of staff, coordination of audits, and oversight of improvement plans for each site

4. Ensures compliance with the Access Matters program including training of staff, coordination of site visits, and adherence to counseling and prescribing guidelines

5. Collaborates with Executive Director in relation to fostering new partnerships with organizations that will result in improved services for patients

Supervision of Staff:

1. Direct supervision of: Associate Director of Primary Care, Primary Care Site Directors, Population Health Manager, Prenatal Team; conducts monthly reflective supervision sessions, performs formal review annually and any performance management.

Fiscal Soundness:

1. Sets productivity standards in collaboration with Executive Director.

2. Interfaces with administrative staff concerning the creation and dissemination of Nurse Practitioner productivity reports and addresses deficits.

3. Ensures staff are adequately trained on billing and coding procedures.

4. Liaisons with all payors in communication with DFO and Billing Department, represents FPCN at local/regional meetings

Qualifications

  • At least 5 years of experience practicing as a Nurse Practitioner
  • At least 3 - 5 years in a Leadership position managing Provider staff
  • Experience in a FQHC or community setting a plus
  • Experience working in underserved communities
  • Master's Degree in Nursing
  • Current Pennsylvania state RN license
  • Current Pennsylvania CRNP license
  • TB, Hep B Vaccination required
  • COVID Vaccination required
  • Current CPR Card required
  • Reliable Transportation
  • Proficiency in Microsoft Office
  • Ability to pass criminal background check
  • Ability to complete credentialing process
