The purpose and mission of this Community Care leadership role is to improve the health and wellness of the community. They will develop and oversee program services and operations across all markets with a focus on program development, program enhancements, managing interdependencies and risks, program status and evaluation, program growth/expansion in both new and existing markets. This role will establish workgroups and work closely with the operational areas to design, develop and implement business processes and project plans to meet business requirements. This role will facilitate development of various reporting tools to monitor operational functions and identify opportunities for improved performance, including development and analysis of reports. They will form partnerships and develop relationships with community organizations and leaders. This role is the subject matter expert that is responsible for direct supervision, accountability and evaluation of an interdisciplinary clinical and non-clinical team that will perform community activities. The leader of this role will be a strategic program leader for our community health and wellness activities across the state. She will develop and maintain the playbook, protocols, procedures, ensure activities are compliant with regulations and safe for patients. She will work with the regional leaders to learn what's working and not working to always improve the playbook. She will work on finding new ideas and activities that can be launched in the regions to improve the health and wellness of those we serve. The leader of the role will be the strategic program leader for our CHW state-wide program. She will develop and maintain program details and playbook. The CHW's will report to her directly for program specific activities and the CHW's will work directly with the regional leaders to be deployed in their regions. Bachelors degree required. 2 years healthcare experience required. 2 years leadership experience required. 2 years community health experience required. 5 years project management experience required.
