Job Overview

The Behavioral Health Care Coordinator (BHCC) works with multidisciplinary teams, psychosocial support providers and self-management teams, managing a specific panel of patients identified as having rising risk or high risk. Responsible for the coordination of care across the system, organizing care activities and sharing information to achieve safer and more effective care. Responsible for enhanced care coordination and care management services, coordinating, facilitating, and integrating mental health treatment, care, and support, which is tailored to meet the specific needs of patient. Ensures that the care provided is patient and family centered, evidence-based, cost-effective, holistic in nature, and leads to better outcomes. Responsible for partnering with the patient and family for seamless transitions in care between the primary care provider, treatment team, hospital, home, school, and community.

Qualifications

  • Education: Master's in Human Services or related field.
  • Experience: Four (4) years in Behavioral Health field; training with an emphasis in pediatric care coordination.
  • Certification(s): American Heart Association Basic Life Support for Healthcare Provider or American Red Cross CPR for the Professional Rescuer or ASHI Basic Life Support for Health Providers
  • Licensure(s): Licensed Social Work (LSW) or Licensed Clinical Social Work (LCSW) or Marriage and Family Therapy (MFT) or Licensed Professional Counselor (LPC) within state of Colorado is required.

This position has been defined and is governed by the Children's Hospital Colorado Medical Staff bylaws, continuation of employment is contingent upon the review and approval of the Colorado Health Care Professionals Credential Application within 3 months of employment. Failure to successfully pass the credentialing process may result in the termination of your employment. Such positions include: Advanced Practice Providers, licensed professional counselors, or Adjunct Medical Staff.

ESSENTIAL FUNCTIONS

  1. Coordinates across multiple sources to help ensure that high needs children have care plans that connect them with appropriate physical and behavioral health services, clearly articulate goals for improved health, identify wrap-around services and supports, and facilitate consistent channels of communication among all parties.
  2. Responsible for complex panel of patients supporting patients/families in their plan of care across all settings (hospital, home, school, community).
  1. Serves as a point of contact, advocate, navigator, and informational resource for patient, family, care team, payers, and community resources. Facilitates related meetings as needed.
  2. Collaborates with patients, families, and providers to influence, and enhance adherence to, an effective plan of care based on mutually developed goals in an environment that fosters shared decision making.
  3. Utilizes the assessment processes that include screening tools in partnership with a multi-disciplinary team comprehensive of patient's families, providers, and guardians; and input from service professionals to identify those members who need. Assesses for patient/family needs in a comprehensive, holistic manner including identifying and addressing unmet social and mental/behavioral health needs.
  4. Uses reports to proactively manage care gaps, over/under utilization, understanding/compliance with Care Plan, and progress towards shared goals.
  5. Provides education and coaching to patients and families to improve health literacy and work towards self-management of their health.
  6. Collaborates with the patients/families to develop a relationship that supports open communication and creation of a care plan that supports the level of support desired by the patient/ family and will lead to optimal outcomes.
  7. Facilitates patient access to appropriate providers as well as other care coordination support, including, but not limited to, external providers, social support programs, appointments, and referrals.
  8. Facilitates transitions in care between the hospital, home, school, and community and ensures that the child's optimal care outcomes are not impacted by the transition.

Complexity: Duties performed involve concepts, theories, and concrete factors to be evaluated and weighed requiring a high degree of analytical ability, independent judgment, and decision-making. Acts as a subject matter expert in the field of expertise.

Decision Making: Exercises judgment and discretion, and is responsible for determining the time, place and sequence of the work performed.

Communications: Contacts are of a remedial nature involving the resolution of problems and where some degree of discretion and judgment are required in carrying out a major program and/or function of the organization.

Supervision Received: Performs work with minimal supervisory oversight. Work assignment is generally unstructured, and employee is responsible for organizing complex, varied, and simultaneous coordination of several functions, programs, or projects in various stages of completion.

Salary Information

Annual Salary Range (Based on 40 hours worked per week): $78,659.24 to $117,988.86
Hourly Salary Range: $37.82 to $56.73
