Registered Nurse PD - Primary Care and Multispecialty Ambulatory Clinic

Under the supervision of the Administrative Nurse Manager, guided by the policies and procedures of the Ambulatory Care Center, and in collaboration with the physician and other clinical staff, the Clinical Nurse PD is responsible for providing safe, efficient nursing care to patients in an out-patient, primary care practice. Triages patients and responds appropriately according to protocol and nursing assessment. The Clinical Nurse PD utilizes the nursing process (Assessment, Planning, Implementation and Evaluation) to provide safe and therapeutic clinical care. The Clinical Nurse PD also provides clinical support to providers and participates as part of the health care team to assure coordination and continuity of patient care. The Clinical Nurse PD is responsible for triage on behalf of the practice both in person and through telephone and electronic communication. S/he participates in the development and implementation of educational programs and materials for patients and their family members. S/he delivers direct patient care to a variety of patient populations. All components of nursing care will be individualized to meet the patient's physiological and psychosocial development. The Clinical Nurse PD will play a critical role in the patient experience including optimizing clinic flow, providing in and outreach to patients and working collaboratively with the team to create a patient centered environment. S/he is responsible for daily clinical operations and maintaining a safe environment for patients and staff. The Clinical Nurse PD consistently projects a professional and courteous demeanor, helps establish and maintain the highest service standards for the practice and complies with the Medical Center House and Telephone Standards. This employee will utilize computer skills in Apex and other UCSF computerized programs. This individual supports the standards of the policies and procedures of the practice and is responsible for maintaining current knowledge of pharmacology and competency in modes of administration and complies with all Ambulatory Services and Medical Center policies and procedures for infection control, safety, and administrative and clinical practice. Embraces and consistently role models the UCSF concepts of values: PRIDE (Professionalism, Respect, Integrity, Diversity and Excellence and AIDET (Acknowledge, Introduce, Duration, Explanation, and Thanks) throughout all interactions with patients, family members, and other employees, both in person and on the phone. The Clinical Nurse PD must have flexibility to cover multiple UCSF medical center primary care and multi-specialty clinic sites.


  • ASSESSMENT

    Identifies the needs of patients through observations of disease symptoms, emotional state, cultural and socioeconomic background and response to medical regimen and treatment.

    Collects and organizes data from the client, encouraging them to present their problems or symptoms through active listening and open-ended questions.

    Assesses each client's problem(s) based on auditory, verbal, and emotional cues, as well as information extracted from the medical record.

    Triages and responds or refers inquiries appropriately. Recognizes and reports specific physical and emotional conditions/needs/concerns of the patients to the appropriate members of the health care team. Alerts physician of unusual observations of patients to facilitate immediate attention, e.g. chest pain, shortness of breath. Demonstrates ability to provide appropriate care based on the needs of a specific individual,

    PLANNING

    Plans as appropriate to the needs of patients and uses knowledge of the cognitive physical, emotional, and chronological maturation process.

    Collaborates with practice providers and all other members of the clinical support team in planning for the clinical support of providers in practice.

    IMPLEMENTATION / EVALUATION

    Provides individualized nursing care which responds to the special needs of the patient population and demonstrates and understanding of the treatment needs required for patients.

    Performs nursing intake process according to the standards in the nursing policies and procedure manual.

    Performs nursing activities as ordered by the physician, including venipunctures and electrocardiograms. Administers medications and thoroughly documents in the medical record. Maintains current knowledge of pharmacology and assure medications are current.

    Instructs patients on specific diagnostic procedures (specimen collection, etc.) Assists as needed with diagnostic procedures and treatments. Maintains knowledge about the operation and care of biomedical equipment in the practice.

    Responds to the emotional needs of patients by demonstrating a sensitivity to their emotional state by relating to patients in an appropriate manner. Refers to appropriate members of the health team as indicated.

    Responds appropriately to emergencies and maintains knowledge of emergency procedures and supplies available.

    Complies with all ACC and Medical Center policies and procedures for infection control, safety, administrative, and clinical practice. Complies with activities mandated by The Joint Commission /Title 22.

    COORDINATION / CONTINUITY / COMMUNICATION

    Organizes nursing care based on the needs of patients, while maintaining the effective flow of patients through the clinic. Adjusts the clinical support activities to reflect changes in priorities during practice sessions.

    Maintains open communication and facilitates continuity of care for patients and/or family with members of the health care team through phone contact, verbal interaction and/or written reports. Utilizes electronic healthcare information (i.e. labs, appointments) to contribute to patient care.

    Assists with coordinating diagnostic testing. Maintains accurate follow-up on results and contacts patient with any significant result requiring treatment ordered by the Physician.

    Answer Nursing telephone line messages. Responds appropriately within the scope of Nursing and recommends treatments under the supervision of a physician and/or use of standardized protocols.

    Maintains open communication with ancillary departments such as pharmacy, sterile processing, EKG and the laboratory to problem-solve issues.

    Actively participates in all staff meetings.

    Demonstrates knowledge of and effective use of communication to resolve questions, problems, concerns, and/or issues.

    Demonstrates courtesy, common sense and follow-through in all telephone communications, maintaining the Telephone Standards as established by the Medical Center staff.

    ADVOCACY

    Provides patients and/or their families with the necessary information to make informed decisions utilizing available resources and makes referrals appropriately. Informs patients/family about services available at UCSF and in the community.

    Informs patients of the Patient Bill of Rights and encourages involvement in their own health care.

    Participates in identifying the educational needs of the all patient populations.

    Provides and reinforces patient health teaching.

    Evaluates the effectiveness of health teaching by obtaining feedback from patients.

    Participates in the review and/or revision of established educational materials.

    SELF-DEVELOPMENT / PROFESSIONALISM

    Assumes responsibility for own career growth, recognizing the need to increase knowledge, stays abreast of current trends and developments within own area of practice, expand skills and show initiative.

    Complies with work-related policies, including license renewal, annual PPD, CPR certification, use of benefits time and dress code, including identification.

    Completion of annual competency testing.

    Ensures a professional and helpful working environment by upholding the House Standards as adopted by the Medical Center staff.

    Contributes to the orientation and learning experiences of other health care providers.

    OPERATIONS MAINTENANCE

    Maintains adequate levels of clinical supplies & laundry.

    Complies with all Ambulatory Services and Medical Center policies and procedures for infection control, safety, administrative and clinical practice. Complies with activities mandated by The Joint Commission/Title 22.

    • Sets up exam room, performs intake process and pre-visit tasks. Provides for physical and emotional

    privacy.

    • Prepares for and instructs patients about specific diagnostic and therapeutic procedures and

    treatments.

    • Assists as needed with diagnostic and therapeutic procedures and treatments.

    • Uses appliances and equipment correctly and maintains safe practice.

    • Obtains specimens and performs Point of Care Testing as directed. Performs QA/QC

    on CLIA waived tests and reports any tests out of QA range to supervisor.

    • Completes annual competency testing as specified by the individual practice.

    • Assists patients with limited mobility to transfer from wheelchair to exam table, chair, or toilet.


Required Qualifications:

  • Minimum of one-year recent experience as a Registered Nurse in an ambulatory practice
  • This position requires flexibilityto orient and work at all UCSF Medical Center locations

Preferred Qualifications:

  • Strongly preferred minimum of one year experience in Pediatrics and/or Family medicine
  • Recent Experience in the use of Electronic Medical Record documentation.
  • Experience with Adult and Geriatric patient populations preferred
  • Ability to work competently and efficiently with Microsoft Office: Outlook, Excel & Work
  • Bachelor of Science Nursing (BSN)

Licenses/Certifications:

  • Current licensure as a Registered Nurse in the State of California.
  • Current American Heart Association healthcare provider CPR certification
