The Community Wellness Nurse serves as a wellness coordinator and ensures delivery of health screenings, flu vaccines and health education at City of San Antonio Senior Centers in the community. This position serves in community-based projects that promote preventative healthcare and positively enhance the lives of individuals.

Primary Responsibilities:

  • Acts as an Ambassador for WellMed at City San Antonio Senior Centers in the community
  • Arrives at work in a timely manner and provides services from 7:30 am to 4:00 pm
  • Performs and completes all program health screenings per required metrics
  • Administers flu vaccines and complies with WellMed and CDC guidelines
  • Coordinates and conducts health education by utilizing WellMed approved educational materials and resources provided by WellMed Clinical Education Department and National Institute of Aging. Will be trained in Evidence based programs and/or ensure appropriate facilitation of health classes and resources by qualified community partners
  • Remains current with WellMed training resources, practices, certifications, and licensures as well as training in evidence-based programs
  • Works within the Code of Professional Conduct, Scope and Standards of Practice, State Law and Regulation as well as following internal policies and procedures
  • Processes appropriate documents and records in an accurate, timely and organized manner
  • Actively seeks ways to improve levels of customer service and quality of care
  • Maintains medical equipment and supply inventory
  • Ensures adherence to proper bio-hazard disposal and safety policies and procedures
  • Ensures deliverables, timelines, and accurate reporting
  • Leverages opportunities to educate consumers about the WellMed Model of Care
  • Performs all other related duties assigned

Required Qualifications:

  • High School diploma or GED
  • Current Texas Vocational Nurse License
  • Current Basic Life Support Certification
  • 2+ years of related LVN experience

Preferred Qualifications:

  • 5+ years of experience in clinical, community or health education setting
  • Proficient computer skills, including Microsoft Word, Excel, PowerPoint and Outlook
  • Excellent organizational and prioritization skills
  • Ability to interact productively with individuals and teams
  • Excellent verbal, written and public speaking skills

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $20.38 to $36.44 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
