Posting Summary
Rutgers, The State University of New Jersey, is seeking a Patient Navigator in the Department of Infectious Disease within the New Jersey Medical School ( NJMS),

The primary purpose of the Patient Navigator position is to provide clinic and community-based HIV, STI testing, and case management services to persons with and at risk for HIV infection. Performs a biopsychosocial assessment and develops an informed care plan to address any identified social service needs and treatment adherence barriers. As part of the team, the Patient Navigator will provide services in clinical settings and on the mobile health unit including working select weekend and evening hours. Maintains the confidential integrity of all client level information. Proactively reports any issues requiring action, intervention, or resolution to the Program Manager in a timely manner.

Among the key duties of this position are the following:

  • Provides a confidential assessment for those clients living with HIV and clients being referred for PrEP services determine the HIV prevention needs of the clients whether their test results are positive or negative; this includes the registration and planning of prevention services.
  • Provides HIV and STI confidential pre and post-test counseling for identified patients and others according to NJ Department of Health and Senior Services and Centers for Disease Control and Prevention guidelines and documents accordingly.
  • Provides counseling and testing services at the clinic and/oroutside events and mobile health unit.
  • Conducts quality controls daily and weekly (e.g. Room temperature, freezer temperature, perform controls etc.).
  • Familiarizes and maintains knowledge of policies and procedures related to all aspects of STI and HIV counseling and testing.

Qualifications






Minimum Education and Experience

  • Bachelor's Degree in Social Work or Human Services plus two (2) years of full-time professional experience with HIV/AIDS.
  • Equivalent education, experience and/or training may be substituted for the degree requirement.

Certifications/Licenses

  • NJ certification as a HIV Counselor.
  • Valid Driver's license required.

Required Knowledge, Skills, and Abilities

  • Must be able/willing to learn to drive mobile unit.
  • Must have completed or be eligible to successfully complete the Rutgers Defensive Driving Training Course.
  • Proficiency in Microsoft Windows based Excel, Access and Word required.
  • .Ability to exercise considerable independent judgment utilizing professional level of knowledge to establish the work priorities within the scope of responsibility.
  • Excellent and demonstrated customer service skills required.
  • Ability to work varied schedules which may include weekends/ evenings.
  • Incumbent must fulfill all required job qualifications including requirements resulting from current/future modifications to federal/state funding guidelines.

Preferred Qualifications

  • Five (5) years preferred.
  • Case management experience and Phlebotomy certification preferred.
  • Bilingual skills. (English/Spanish) preferred.
