Job Summary
Under the direction and supervision of the Registered Nurse, assists the professional staff by performing a variety of paraprofessional and technical functions which contribute to the safety, care and comfort of neonates, infants, children, adolescents, adults and geriatric patients and their families. Provides direct and indirect patient care, including technical skills and procedures, performs routine secretarial and clerical duties and participates in maintaining the environment.
Essential Functions
- Provides patient care in a manner, which supports the patient's rights and dignity.
A. Provides care in a considerate and respectful manner. B. Maintains confidentiality of patient information. C. Ensures patient privacy by providing appropriate screening and draping during procedures and care activities. D. Refers questions from patients and families to appropriate personnel. E. Respects the rights of patients and families to make informed health care decisions. - Meets hygiene and grooming needs of patients.
A. Assists patients with bed baths and bathes selected patients unable to bathe themselves. B. Assists patients with tub baths or showers. C. Provides mouth, skin, hair, nail and perineal care. D. Shaves patients as directed. E. Administers routine AM and HS care to all assigned patients. F. Assists patients in the use of ADL devices. - Assists in meeting the fluid and nutritional needs of patients.
A. Positions patients for meals. B. Delivers and sets up tray. C. Provides for patient hygiene before and after meals. D. Feeds patients or assists with feeding as required. E. Provides nourishment between meals as ordered. F. Provides fresh drinking water. G. Accurately records intake as ordered and records totals at end of shift. - Participates in maintaining patient's skin integrity.
A. Turns and positions patient at least every two hours during bed rest. B. Inspects skin for signs of redness, swelling, and bruising or abrasions and reports findings to RN. C. Utilizes appropriate pressure relieving devices. D. Assists with pressure ulcer care in accordance with written protocols. - Initiates measures to provide for patient and family comfort.
A. Maintains a quiet environment. B. Provides for patient warmth as appropriate to the season, environmental temperature, age requirements and patient's condition. C. Makes occupied and unoccupied bed and assures linens are clean and dry. This includes soiled linen disposal. D. Behaves in a supportive manner that reduces anxiety and stress. E. Provides for patient needs or notifies appropriate person. - Assists patients with elimination needs.
A. Responds promptly to requests for assistance with urinal, bedpan or commode. B. Assists patient to bathroom as necessary. C. Correctly applies and cares for condom catheters. D. Provides Foley catheter care. E. Empties urinary drainage bags. F. Provides routine stomal care as per policy G. Provides appropriate patient hygiene after toileting. H. Accurately measures and records: Urine output - Nasogastric tube output - Chest tube output - Other output as directed I. Accurately totals output at end of shift. - Assists patient with pulmonary toilet.
A. Performs chest physiotherapy as directed. B. Assists in setting up equipment for oxygen therapy as ordered but does not administer. C. Provides physical care to patients on ventilatory support. - Provides and maintains a safe environment through preventive and intervention measures.
A. Utilizes age appropriate restraining devices such as cribs and side rails as directed by RN. B. Provides appropriate assistance and periodic observation to prevent falls. C. Immediately reports unsafe conditions. D. Adheres to policies and procedures regarding restraints. E. Performs equipment checks as assigned. Removes broken equipment immediately. F. Sets and/or verifies equipment alarms and alarm volume as directed by RN. G. Provides Constant Observation or Periodic Awareness as directed by RN. H. Participates in hourly patient rounding I. Monitors patient care equipment to assure alarms are in "ON" position. J. Immediately alerts professional staff when equipment alarm is activated. K. Responds to patient call lights promptly. L Adheres to medical center's infection control, safety, fire and disaster policies and procedures. - Collects patient data through observation, measurement and data retrieval.
A. Obtains and records accurate vital signs including temperature, pulse, respiration and blood pressure. B. Obtains and records accurate height and weight on admission, discharge and periodically as ordered. C. Places cardiac and respiratory monitoring leads as directed. D. Performs 12-lead EKG and obtains rhythm strips as directed. Records, initials and reports completion to the ciinician. E. Performs bedside glucose monitoring in accordance with written policy and procedure and records and reports results. F. Applies pulse oximeter and monitors, records and reports readings 90% or below to clinician. G. Documents data in accordance with departmental policy and procedure. H. Reports observations to RN in a timely manner. - Obtains specimens for laboratory analysis.
A. Performs venipuncture to obtain blood specimens for laboratory analysis. B. Collects urine, stool and sputum specimens for analysis and culture. - Performs routine laboratory studies
* Performs studies including but are not limited to the following: Point of Care Urinalysis, POC Urine Pregnancy. POC Hemoccult, POC Rapid Strep - Performs technical skills as defined by scope of practice and competency assessment process.
A. Assists the professional staff with diagnostic and therapeutic procedures. B. Sling application C. Measurement for crutches - Participates in the implementation of the treatment plan developed by the professional staff, patient and family.
A. Assists other staff members in providing patient care. B. Completes assignments within established time frames, which includes CIS/EPIC ASAP worklists and other assigned tasks. C. Throughout the shift on CIS/EPIC ASAP: reviews, updates and completes documentation on worklists. D. Throughout the shift on NO - CIS unit: reviews, updates and completes documentation on the appropriate form and/or Medical Record. - Communicates effectively with patients, families and staff.
A. Communicates with patients and families in a respectful and therapeutic manner. B. Explains paraprofessional procedures and treatments to patients and families. C. Communicates observations of patients' status to professional nurse and other members of the health care team including change of shift report. D. Documents accurately data, observations and interventions on appropriate departmental forms. E. Refers to medical record as necessary. F. Asks questions and seeks clarification as necessary from the RN G. Interacts with coworkers in a respectful manner, whichpromotes team building. H. Shares acquired knowledge and learning. I. Reports on and off duty to charge RN. A. Completes assignment within established time frames. B. Ask questions and seek clarification as necessary. C. Escalates difficult situations as appropriate. D. Initiates CPR as required. E. Provides assistance during emergency procedures as directed. G. Provides post mortem care as required. - Provides transportation services, which support patient, care activities.
A. Accompany/transports patients to diagnostic and treatment areas and upon admission transfer or discharge. B. Transports patient's medical record to designated hospital locations as directed. C. Transports supplies, specimens and equipment as directed. - Performs activities to maintain a clean, safe and therapeutic environment.
A. Cleans and prepares patient treatment rooms. B. Cleans stretchers and other patient care equipment as directed. C. Orders, replaces and/or restocks supplies and equipment as necessary. D. Cleans and straightens utility rooms and treatment areas. E. Assists and maintains the unit's environment instate of regulatory readiness. - Is responsible and accountable for own actions.
A. Adheres to medical center's human resource policies and procedure including departmental dress code and standards for attendance and punctuality. - Participates actively as a member of the healthcare team.
