Job Description:
- The Stable Families, Thriving Futures Facilitator/Navigator plays a key role in implementing a Department of Labor-funded grant focused on family stability and community health. This position conducts eligibility assessments, facilitates educational sessions, coordinates participant case management, and supports data collection and reporting. The role involves collaboration with internal departments and external partners, community outreach, and health education delivery. Flexibility is required for evening and weekend events. Emergency response participation is expected. Strong communication, organizational, and facilitation skills are essential.
Certifications:
- Driver's License (Required).
- CPR (Required within 60 days).
- AED (Required within 60 days).

Overview Statement Of Position Duties:
- Under the direction of the required provisions of the Department of Labor grant reporting to the FCHD CHD Division Project Manager (Management Analyst III), the Stable Families, Thriving Futures Facilitator/Navigator:
- Performs eligibility assessments, analytic work in data analysis, and reports on multi-partner activities and business processes, including developing, recommending, and implementing eligibility activities and programs for improvement in the performance, quality, compliance, or efficiency of the program services. Ensures adherence to grant processes and outcomes. Participates in classroom fidelity to implementation, data collection efforts, liaising with county stakeholders, internal FCHD department leads, grant partners, and subcontractors. Assists implementation efforts of the Stable Families, Thriving Futures grant to include but not limited to conducting program needs assessments, interviews, and evaluations. Responsibility for assigned program participant case coordination, classroom facilitation, co-parenting navigation/coaching, administering program intake application, pre- & post-assessment group surveys, and providing or completing instruments into central program data collection. Participates in coalition and advisory committee convening and facilitation, new partner engagement and recruitment, community awareness, outreach and recruitment, training coordination, and other duties as assigned.

- Office worker who interacts regularly with community members. Engages with community members at events, sometimes within six feet. Engineering/Administrative/Work Practice Controls: Re-arranging of meeting rooms and other public spaces; Facilitating virtual and in-person educational support groups.
Role And Responsibilities:
Program Operations:
- Works in collaboration with the CHD Division Grant Project Manager and Lead Facilitator Human Service Worker III to achieve all Stable Families, Thriving Futures Project grant performance requirements. Performs professional-level work. Adheres to the implementation of the grant evaluation plan, working with evaluation contractor(s) if necessary. Supports the coordination of performance efforts with other existing performance management efforts countywide. Supports performance measurement activities, including data collection, among program participants and grant partners. Participates in evaluation and assessment activities to ensure program quality, determine unmet needs, or ensure efficacy of existing policies, processes, or programs. Contributes to grant performance reports, annual reports, and presentations. Attends Stable Families, Thriving Futures Project Department of Labor required trainings and meetings. Identify and schedule trainings to achieve grant outcomes. Prepares reports and presentations of analysis and findings.
- Programmatic Policy Analysis and Development: Reviews existing policies and procedures and makes recommendations based on best practices review, program observations, and input from client participants, staff, and relevant stakeholders. Makes recommendations regarding operational tools and approaches used by staff. Reviews regular reports on work performed, outcomes, and recommendations for future enhancements. Reviews surveys and/or studies, including review and evaluation of preliminary findings.
Functional Partnerships:
- Identifies and engages new community partners to support the achievement of grant outcomes. Establishes and maintains collaborative relationships with public, private, interfaith, non-profit, academic, and other human service providers to support the establishment of functional partnerships. Supports information sharing approaches to assure two-way communication about participants, program concerns, and related information with health department staff, diverse community partners, and other human service providers. Supports feedback loops with staff, other workers, and participants to continuously improve the program. Supports efforts to strengthen strategic partnerships. Seeks to build partnerships with formal and informal community leaders and champions to advance the Stable Families, Thriving Futures Project goals and objectives within the multi-cultural and ethnic populations. Develops and disseminates outreach and educational materials to targeted community groups/members using various methods of distribution, in coordination with the OEU team. Recruits targeted individuals for program participation.
Community Health Education:
- Using population-appropriate learning concepts and culturally informed dialogue, education plans and curricula, develop and/or deliver health education curriculum-based programs to specific target populations with emphasis on responsible parenting, understanding trauma and building resiliency, co-parenting, financial capability, health and wellness topics, and emerging public health issues. Prepares and/ or gathers public health education written and audiovisual materials for specific target groups. Measures the effectiveness of the education efforts. Identifies and works with sub-groups of the community, to include, but not limited to: Hard to reach populations including the medically underserved and individuals without a primary care provider, various ethnic communities and non-profit organizations that routinely serve identified ethnic communities, small business owners, non-profits and FQHC's, health care providers and their staff, interfaith houses of worship and their congregations.
- Coordinates with other internal agencies, county-wide agencies, and external staff and programs so that opportunities for participants can be maximized. Builds capacity for health education in the community. May participate in the recruitment process and recommend the hiring of selected staff. May participate in other panel interviews as requested.
Development of Self and Others:
- Identifies learning needs, discusses with supervisor, and initiates a plan to meet those needs. May also attend learning opportunities recommended by supervisor. Attends required agency trainings and meetings (i.e., CPR, OSHA, etc.). Attends services and workshops to stay informed on public health and emergency management best practices and shares pertinent information with peers. Keeps informed of agency notices, changes in procedures and policies, and county communications.
- Attends weekly/biweekly meetings with peers and/or supervisor to share information, obtain information on procedural or policy changes, discuss ideas to improve delivery of service, or problem-solve workflow issues, and/or assign work and review action taken on past assignments. Develops professional/job-related networks that support and strengthen job performance. Functions as a team member in providing customer-focused services.
- Public Health Emergency Response/First Responder Maintains working knowledge of emergency plans, with an emphasis on understanding the duties and responsibilities of their position during an emergency. Completes all required emergency preparedness trainings; participates in additional trainings and exercises as needed. Maintains up-to-date contact information in the county emergency alert system (EAN), including after-hours contact information (e.g., home or cell phone), and responds to emergency alerts as specified. Responds to public health emergencies, as directed, and effectively carries out emergency duties as assigned.
