Summary:
Performs a wide variety of skilled physical therapy treatments for patients based upon the physical therapy evaluations and goals completed by the physical therapist. Advances the patient's treatment plan in response to the patient's progress. Assists with discharge planning.

Responsibilities:

*Provides skilled physical therapy treatments to a wide variety of patients. Designs and teach exercise program to meet PT goals. Fits and teaches patient to use ambulatory devices including braces, walkers, canes, and crutches safely to negotiate stairs, curbs, and unlevel ground. Utilizes appropriate safety equipment with patients (gait belt, mechanical lift, body mechanics, etc). Instructs patient and family in home exercises, ambulation, and use of adaptive equipment for discharge.
*Contributes to various interdisciplinary team meetings to plan for patient needs, discuss family education plans, and D/C plans. Coordinates outpatient services, when necessary, and when skilled home health services are complete. Responds to patient concerns in a timely and professional manner.
*Documents patient treatment and progress toward goals, and reports to the Clinical Manager all significant incidents to self or patient. Accurately sets priorities considering patient assignments, time and resources.
*Utilizes and provides appropriate supervision to students, and support personnel involved with clinical tasks.
*Abides by professional organization code of conduct during all patient interactions. Represents the department or participates in program development, marketing, or approved community activities.

Qualifications
Required
*Associate's Degree in a Physical Therapy Assistant program.
*Physical Therapy Assistant license required by first date of care.
* Basic Life Support (BLS) Certification
Preferred
*Experience in outpatient, inpatient, home health, and/or long term care.
