Job Description

Corewell Health South is seeking an experienced APP who is interested in a Full-Time opportunity at our newest walk-in clinic at Southwestern Medical Clinic in Bridgman.

Will consider Nurse Practitioners and Physician Assistants. Our Advanced Practice Providers work 3 - 12-hour shifts per week. The Bridgman location will be open Monday through Saturday, 7am - 7pm. Providers typically see 30 - 40 acute visits per 12-hour shift. If you are looking for a fast paced environment and a 3 day work week, this may be the opportunity for you.

No call required. Fluency dictation, Abridge, an Ambient AI assisted program which captures patient visits in real time and then assists with creating notes and after visit summaries available, Epic EHR.

In collaboration with or under the supervision of the Medical Director, Supervising Physician and Practice Manager, provides primary and/or urgent health care services and consultation for patients and families, focusing on wellness and health promotion. Serve as a leader, educator, and researcher for patients, their families, and the community.

Essential Functions
  • Provide comprehensive primary and/or urgent care within the regulatory guidelines set by the State of Michigan, Rural Health Clinic (RHC) Code of Federal Regulations, Joint Commission (JC), Patient Centered Medical Home (PCMH) and/or other regulatory agency including performing diagnostic and therapeutic procedures, prescribing and administering medical treatment and medications; plan and coordinate patient care; periodically reviewing the clinic's patient records with the collaborating physician if applicable.

  • Perform complete screening and routine examinations as well as assessments, devising plan of care in order to determine a baseline, reflect the nature of the patient visit, diagnose and promote good health. Perform, order, and interpret appropriate laboratory test; notifies patients of results as needed.

  • Assess information and/or symptoms of patient by phone or other technology in order to prescribe treatment or refer to appropriate resources.

  • Consult and instruct patients in order to provide support, facilitate recovery and foster patient self-care at home, promotes health through continued follow-up care.

  • Arrange for or refer patients to needed services that cannot be provided at the clinic. Assure that adequate patient health records are maintained and transferred as required when patients are referred.

  • Participate in the development, execution and annual review of the written policies governing the services the clinic furnishes as appropriate. Perform other duties as needed and appropriate to the scope of practice.

  • Committed to the continuous improvement of the patient experience which includes patient safety, quality of care and excellent service. Adhere to the organization's published values.

  • Participate in the clinic quality improvement activity.

    Qualifications
    • Master's Degree MSN-Nurse Practitioner. Required

    • Preferred Doctorate

    • 1 year of relevant experience current, relevant clinical experience as nurse practitioner Preferred

    • LIC-Registered Nurse (RN) - STATE_MI State of Michigan Upon Hire required

    • LIC-Nurse Practitioner (NP) - STATE_MI State of Michigan Upon Hire Required, or

    • Master's Degree in Physician Assistant Studies Required

    • LIC_Physician Assistant (PA) - STATE_MI State of Michigan Upon Hire Required

    • CRT-Basic Life Support (BLS) - AHA American Heart Association Critical Care (CCU), Intensive Care (ICU), Emergency Department (ED), Cardiology, Trauma Care, Inpatient Rehab, Interventional Radiology, Anesthesia, Primary Care Dowagiac, Primary Care Hartford, Primary Care Coloma, Primary Care and Walk In Clinic Watervliet, SWMC Niles, ENT Niles and SWMC Stevensville Walk In Clinic. 90 Days Required

    • CRT-Adv Cardiovascular Life Support (ACLS) - AHA American Heart Association Upon Hire Preferred

    • CRT-Pediatric Adv Life Support (PALS) - AHA American Heart Association Upon Hire Preferred
