The Home Health and Hospice Program Coordinator coordinates patient assessments, gathers data for analysis, develops and implements processes to improve the overall quality and efficiency of patient care for the designated department and population served. Provides direct and indirect patient care as needed; oversees the delivery of patient care; and provides day to day program management to coordinate the delivery of safe, high quality patient care.

regulatory requirements. Serves as a resource and mentor to staff and team members.

This Oncology Program Coordinator position works within our Outpatient Rehab departments. Travel between outpatient locations, as needed.

Oncology Program Coordinator:

  • Organizes, formulates and coordinates a multi-disciplinary approach to the patient population forachievement of optimum outcomes.

  • Reviews referrals to ensure appropriateness of admission/appointment; criteria is met; patient discharged with a plan for transition of care, appropriate documentation of patient education and instructions; screening of referrals to ensure appropriate patient placement in programs; ensuring patients and/or family members understand the plan of care; preparing the patient and family for ongoing post discharge care; and responding to patients' needs and concerns.

  • Facilitates effective outcomes and resolves complaints, issues, or concerns.

  • Plans, implements and coordinates strategies through evidence-based practices to support quality patient care; and improve outcomes (e.g.. length of stay, turn-around time, resource consumption).

  • Develops, collects, analyzes and evaluates data as needed to identify trends. Presents findings and makes recommendations for performance and quality improvement, Joint Commission/regulatory agency requirements, and tracking patient outcomes aimed at accomplishing department and organization goals.

  • Advocates for the patient, protector of patient's rights, and provider of psychological/social/cultural support.

  • Integrates patient's individual religious/ spiritual, ethnic and cultural beliefs into the plan of care.

  • Develops, implements, coordinates and provides continuing education for the department, staff, patients, and families, as well as protocols to support standardization of care.

  • Provides education. Education is specific to the need of the patient and well documented.

  • Education is incorporated into the plan of care and includes physicians, staff, population specific representatives regarding patient care options.

  • Serves as a liaison between department staff, patients and families, physicians, external vendors/partners to ensure a collaborative approach to care for patients and families.

  • Age specific admission and ongoing assessments are completed, documented and used to develop a plan of care.

  • Coordinates discharge of the patient from one environment to the next. Coordinates discharge / ongoing care needs with ancillary services as appropriate.

  • Communicates overall plan of care to direct care staff, as well as changes in plan of care during the course of treatment.

  • Acts as a resource for peers, the organization, families and community members.

  • Organizes educational presentations, exhibits, and community events for professionals and the community; and represents Carilion Clinic at conferences and professional meetings.

  • Promotes programs and arranges exhibits at various professional presentations, health and career fairs and community events in collaboration with the management team and Strategic Development.

  • May schedule appointments; answers calls; collects, maintains, and analyzes data; identifies trends and develops documents, spreadsheets and other tools as applicable to the department population.

  • Supports the orientation of new employees; assesses and implements unit educational needs and performs other duties as designated by the department.

  • Participates in department and system performance improvement initiatives.

  • Evaluates the effect of care coordination and interventions on quality outcomes.

  • Performs concurrent medical record reviews in assigned area. Collects and analyzes relevant patient data in accordance with accepted Disease Guidelines. Analyzes and evaluates the effect of care management on quality outcomes and fiscal parameters.

  • Documents utilization review in accordance with departmental guidelines.

  • Actively participates in quality improvement projects.


What We Require:

Education: Graduate from an accredited PTA/COTA program.

Experience: Minimum of 5 years' experience as a therapy assistant.

Licensure, certification, and/or registration: Current discipline specific certification as PTA or OTA and Virginia license. Valid Virginia (or state specific) driver's license required for employees working in Home Health or departments where the work requires traveling.

Other Minimum Qualifications: Effective interpersonal, communication, and team-working skills required. Strong critical thinking, conflict resolution, organization and effective judgement abilities required. Must successfully complete orientation and competency validation requirements. Must be able to meet essential job functions. Designated areas may require Knowledge of Medicare/Medicaid and other third-party payor regulations
