Description Job Title: CCS Service Facilitator Company: Lutheran Social Services of Wisconsin and Upper Michigan Location: Columbia County, WI (Remote & Community-Based) Job Type: Full-Time Overview: Lutheran Social Services (LSS) is hiring a Service Facilitator for our Comprehensive Community Services (CCS) program in Columbia County. This role supports individuals with mental health needs through community-based care coordination across home, school, and other environments.
