 Responsibilities: 
 
 
 Conduct outreach and assessments in the community (Greater Lowell and Merrimack Valley Region)Evaluate and assess referrals using a Housing First frameworkCollaborate with local homeless service providers to facilitate smooth transitions from shelters to permanent housingMaintain accurate case records and collect data for statistical analysisDevelop and implement participant-centered service plans, focusing on achieving participant goalsMonitor participant progress, identifying strengths, needs, barriers, and goals to sustain housing and increase self-sufficiencyEducate participants on daily living activities and refer them to necessary service providersConduct home visits and assist participants in accessing community resourcesNetwork and maintain positive relationships with landlords, property managers, housing authorities, social service organizations, and state agenciesProvide crisis intervention, mediation, and service coordination as neededCollaborate with internal and external partnersParticipate in regular meetings and training sessions
 
 Qualifications: 
 
 
 High School Diploma or equivalent requiredRelevant experience in case management or a related fieldKnowledge of community resources for low-income individuals and familiesEmpathy and understanding of homelessness and related issuesExcellent customer service skills and ability to communicate effectively with supervisors, coworkers, volunteers, donors, and participantsAbility to manage multiple priorities in a fast-paced environmentProficient in basic computer software and data collection systemsStrong crisis management skillsAbility to maintain accurate records and confidentialityBilingual (English/Spanish, English/Haitian-Creole, English/Khmer, English/Portuguese) preferred
