Summary

The primary functions of the Intake Coordinator position are processing the intake of patients and the development and management of referrals of patients. The position also interacts with potential patients and referral sources explaining services as well as tracks and monitors cancellations for quality of care and business development.



Responsibilities

  • Fields incoming referrals and monitor all channels for referrals, including phone, e-mail, faxes and EMR, and respond in a timely manner.
  • Communicates with all outside agencies to address any concerns with the referral process.
  • Identifies and informs prospective patients and their families about the nature of ACHC's services and set expectations for the delivery of those services; updates referral sources on status of referrals.
  • Assesses new patient criteria for interested patients, including whether or not they are in a qualified area, have acceptable insurance and are either homebound or home limited.
  • Initiates new patient protocol and schedules first appointment with provider for qualified new patients; enters all patient data into electronic medical records (EMR) and updates provider.
  • Generates various letters periodically to patients who have been discharged from the practice; close out accounts on patients who are no longer active with service.
  • Completes special projects as assigned.
  • Attends Company and industry-related meetings, as required.
  • Establishes individual goals which are aligned with the Company's business strategies and objectives.
  • Functions as a contributing and motivating member of the department's team and other teams, as assigned.
  • Performs other duties as assigned by executive leadership.



Education and Experience

Education Requirements



  • High School Diploma or GED
  • 2+ years of Healthcare Industry related experience



Knowledge & Experience Requirements



  • Proficient in Microsoft Office products
  • Excellent understanding of the mission of ACHC
  • Excellent communication skills
  • Team oriented
