Job Summary:

The Community Partnership Specialist III assists with the identification of populations, needs, trends and patterns that may impact outcomes, experience, and cost of care through in-depth analyses and prioritization of SDOH-related strategies that drive social transformation then develops and fosters new working relationships with community stakeholders/partners in targeted geographical communities, and oversees the partnership development process.

Essential Functions:

  • Maintains working relationships with existing community stakeholders/partners, including providing high-level support to drive the overall satisfaction and engagement of key community partners
  • Provides support to the Life Services member-facing team as it relates to developing understanding of community dynamics within targeted geographies, helping the team to maintain a higher understanding of the members in the region
  • Coordinates activities for Life Services team to engage with current and potential members, to support recruitment within targeted geographical communities
  • Provides research and analysis on the unique needs, challenges, strengths, and resources within a community to allow Life Services to best serve members, and makes strategic recommendations to leadership accordingly
  • Develop and maintain expertise in Federal and state specific requirements that impact and the social determinant of health model, and recommend innovative standardized population health strategies while maintaining a disciplined approach when addressing market specific nuances
  • Collaborates with Market leadership to evaluate opportunities for improved members experience with focus on measurable benefits
  • Keeps leadership informed of community partnership statuses, and opportunities for relationship enhancement
  • Support the management of financial investments made by the Market in to ensure all fiscal outcomes are achieved and connections of business needs to the providers.
  • Understands, gathers, and utilizes data as it applies to targeted geographic regions that are being served within the Life Services program
  • Serves as a primary contact person for Life Services in targeted geographical communities, as needed
  • Performs any other job-related instructions, as requested

Education and Experience:

  • Bachelor's Degree in Public Administration, Political Science, Urban Affairs, Social Work, or related field, or equivalent years of relevant work experience is required
  • Master's Degree preferred
  • Minimum of five (5) years of Community Development experience is required
  • Previous experience in working with Medicaid population is preferred

Licensure and Certification:

  • Employment in this position is conditional pending successful clearance of a driver's license record check. If the driver's license record results are unacceptable, the offer will be withdrawn or, if you have started employment in this position, your employment in this position will be terminated
  • To help protect our employees, members, and the communities we serve from acquiring communicable diseases, Influenza vaccination is a requirement of this position. CareSource requires annual proof of Influenza vaccination for designated positions during Influenza season (October 1 - March 31) as a condition of continued employment. Employees hired during Influenza season will have thirty (30) days from their hire date to complete the required vaccination and have record of immunization verified.

Working Conditions:

  • Mobile Worker: this is a mobile position, meaning that regular travel to different work locations is essential. Will be exposed to weather conditions typical of the location and may be required to stand and/or sit for long periods of time
  • Perform reasonable travel related duties as needed to ensure administration of the program
  • Required to use general office equipment, such as a telephone, photocopier, fax machine, and personal computer

Compensation Range:
$61,500.00 - $98,400.00 CareSource takes into consideration a combination of a candidate's education, training, and experience as well as the position's scope and complexity, the discretion and latitude required for the role, and other external and internal data when establishing a salary level. In addition to base compensation, you may qualify for a bonus tied to company and individual performance.
