Make it happen at HOPKINS. Suburban Hospital is a 228-bed, Level II Trauma Center, and a community-based hospital in Bethesda, Maryland. Suburban Hospital is a member of Johns Hopkins Medicine and has served Montgomery County and the surrounding area for more than 65 years. The designated trauma center for Montgomery County, Suburban Hospital is fully accredited by the Joint Commission and has earned ANCC Magnet Recognition for nursing excellence. What you will do: The Care Coordination Registered Nurse (Case Manager) evaluates timeliness and appropriateness of patient care, and coordinates discharge or transition planning for a smooth progression to post-hospital care. Utilizing a collaborative process in conjunction with Care Management Coordinator, Social Workers and other health care team members, the Case manager will assess, plan, implement and evaluate the options and services required to meet an individual's health and health related needs. The Care Manager serves as an advocate for the patient, family, and organization to promote quality, cost effective outcomes. Active participation in program development and process improvement efforts is an expectation of the role of the Care Manager. What awaits you:

Salary Range: Minimum 53.02/hour - Maximum 53.02/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
