*Hiring For Care Management Support Coordinator !*
*100% REMOTE* *Note: Candidates from EST time zone can only apply!* *Position Purpose:* Supports administrative care management activities including performing outreach answering inbound calls and scheduling services. Serves as a point of contact to members providers and staff to resolve issues and documents member records in accordance with current state and regulatory guidelines. Provides outreach to members via phone to support with care plan next steps community or health plan resources questions or concerns related to scheduling and ongoing education for both the member and provider throughout care/service Provides support to members to connect them to other health plan and community resources to ensure they are receiving high-quality customer care/service. *Day to day task!* *Outreach members telephonically (about 30-40 calls a day) *Answer incoming calls from members. *Create Case, Create Tasks, Create Referrals in member's chart in TruCare *Complete screenings in TruCare *Filter Excel Spreadsheet *Utilize Outlook Calendar for daily planning. *Document in TruCare *Handle 80-100 calls per week (outbound and inbound calls) *Complete screenings on all new members *Document all member interactions and outreach in Trucare *Education/Experience:* Requires a High School diploma or GED Requires 1 - 2 years of related experience *Shift: *Training 8AM-5PM EST 4 weeks Work schedule - 8AM-5PM EST Mon-Fri - No OT *Experience Level* Intermediate Level *Pay and Benefits* The pay range for this position is $24.00 - $24.00/hr.
