The Care Integration Manager - Remote is responsible for leading and managing the Community Care Coordinator (CCC) program, driving patient access to high-quality O&P services across Hanger Clinic's nationwide network. This role focuses on developing and executing strategies that enhance patient engagement, streamline referral processes, and improve access to care. The Care Integration Manager will ensure consistent training, operational excellence, and measurable outcomes for the CCC team, aligning with Hanger Clinic's mission to empower patients through personalized care.



Your Impact

    Manage the CCC program to meet national patient access goals and improve engagement.
  • Create and implement consistent training for CCCs to enhance service quality.
  • Establish and track KPIs for CCCs, using data to drive improvements.
  • Streamline referral and onboarding processes to enhance patient access.
  • Work with clinical and operational leaders to align the program with patient and partner needs.
  • Analyze data to inform decisions and improve patient access strategies.
  • Guide CCCs in building local healthcare partnerships to increase referrals.
  • Maintain program compliance with Hanger Clinic's care standards.


Minimum Qualifications

Minimum

  • Bachelor's degree in healthcare administration or a related field, or equivalent experience.
  • At least 3 years of experience in program management or patient access roles.
  • Must have, or be eligible to obtain, a valid driver's license and driving record within the standards outlined within Hanger's Motor Vehicle Safety Policy and Procedures.

Preferred

  • Experience in developing and managing patient access programs within the healthcare industry.
  • Knowledge of referral management systems, CRM platforms, and data analysis tools

  • Experience developing and implementing training programs, with the ability to standardize practices across diverse teams.
  • Excellent communication and organizational skills, with the ability to manage multiple priorities and stakeholder needs.
  • Ability to collaborate effectively with clinical and operational teams to align patient access strategies with organizational goals.
  • Strong analytical skills and attention to detail, particularly in tracking performance metrics and improving program outcomes.

#LI-Remote

Pay range of $80,000 to $115,000 annual salary + annual bonus: up to 10% of base pay depending on bonus criteria. This pay range is posted to comply with wage transparency laws. Hanger salary ranges vary based on skill, ability, knowledge, geographic location and other variables.
