Hospital at Home is an innovative program that combines in-home and virtual visits with remote monitoring, offering patients timely, hospital-level care in their homes. Though the list of diagnoses enrolled in Hospital at Home continues to expand, common disease states include acute pneumonia, cellulitis, COVID-19, urinary tract infection, dehydration, or exacerbations of chronic conditions, such as heart failure or chronic obstructive pulmonary disease. For many patients, the program provides an alternative to the traditional hospital setting and results in improved patient outcomes and satisfaction. The Hospital at Home team is looking for motivated nurses with critical thinking skills, acute care experience, and an interest in serving the Parkland community.

Primary Purpose

Physicians in the Hospital-at-Home service line are responsible for attending-level management of patients with acute medical conditions, providing medical oversight of Hospital-at-Home Advanced Practice Providers, and developing, implementing, and monitoring quality standards of care within the Hospital-at-Home specialty area.

Minimum Specifications

Education

- Must have successfully completed an Accreditation Council for Graduate Medical Education (ACGME) or American Medical Association (AMA) approved residency for appropriate specialty

Experience

None

Equivalent Education and/or Experience Certification/Registration/Licensure

  • Must be currently licensed for the practice of medicine in the State of Texas
  • Must meet and maintain licensing and registration requirements as delineated by the Medical Staff Bylaws, Medical Staff Rules and Regulations, and Credentialing Policies/Procedures, if any
  • Must have a valid Texas Driver License or obtain a Texas Driver License within 30 days of employment

Skills or Special Abilities

  • Must be able to demonstrate a working knowledge of inpatient medical assessments and treatments
  • Must demonstrate basic technical proficiency required to access electronic medical record, conduct video and audio telehealth visits, participate in remote physical examinations, and interact with technology required to provide acute care in the patient home setting
  • Must be able to work effectively with other health care professionals and serve as a resource to the community
  • Must be able to demonstrate patient centered/patient valued behavior
  • Preferred: Spanish proficiency is preferred

Responsibilities

Patient Care

  1. Conduct initial and ongoing assessments to obtain accurate information about patients.
  2. Collect collateral interviews and medical documents essential to patient care.
  3. Engage in shared decision making to plan diagnostic and therapeutic interventions.
  4. Develop and direct the implementation of treatment plan throughout the hospitalization.
  5. Document information relevant to the care of patients in a professional and timely manner.
  6. Develop competency in virtual care environments, including, but not limited to, telephone encounters, video encounters, and use of peripheral devices and software used in the care of remote patients.
  7. Conduct in-home assessments within the Hospital-at-Home catchment area as required to advance medical and social care needs of patients.
  8. Work with multidisciplinary team to plan safe discharge and transition to post-acute services.
  9. Counsel and educate patients and their social support system regarding health promotion, appropriate management of acute conditions, and self-management of chronic conditions.
  10. Coordinate care with health care professionals at Parkland Health or within the community including specialty care and allied team members.

  • Provide quality medical and systems education to learners rounding with the service line.

  • Maintain medical knowledge to achieve the desired practice outcomes.

  • Stay abreast of the latest developments, advancements, and trends in the field of medicine by attending seminars/workshops, reading professional journals, actively participating in professional organizations.
  • May participate in writing and/or presenting publications and presentations.

  1. Practice cost-effective health care.
  2. Utilize established clinical practice guidelines and referral protocols to effectively manage care within the system.
  3. Advocate for patient care within the system and community.
  4. Assist patients with system complexities, including acting as a patient advocate within the system.
  5. Identify and recommend appropriate process changes to improve effectiveness, productivity, and efficiency in support of overall goals of the unit and Parkland.
  6. Maintain knowledge of applicable rules, regulations, policies, laws and guidelines that impact the area.
  7. Develops effective internal controls designed to promote adherence with applicable laws, accreditation agency requirements, and federal, state, and private health plans.
  8. Seeks advice and guidance as needed to ensure proper understanding.
