Position Overview
The MDS Nurse Assessor is responsible for collecting and submitting assessment data critical to the health and well-being of our residents. You will support the MDS/PPS Coordinator in completing PPS and/or OBRA assessments for both subacute and long-term care residents, as well as perform additional tasks as assigned.

Responsibilities
RN - MDS Nurse Assessor - Essential Job Functions:
Accurately and timely complete and submit Minimum Data Set (MDS) assessments Ensure compliance with Federal, State, and facility policies Actively participate in utilization management processes, including PDPM and state case mix systems Conduct resident interviews Compile, monitor, and audit clinical documentation Communicate clinical issues effectively to appropriate clinicians Support and assist the MDS/PPS Coordinator as needed Demonstrate thorough knowledge of policies and procedures Utilize electronic timekeeping systems as directed Follow facility dress code and identification policies Abide by all policies related to electronic devices, computers, and tablets Uphold Elderwood's Mission, Vision, and Values Maintain compliance with company code of conduct, HIPAA, and regulatory standards Perform other duties as assigned by supervisor or Clinical Reimbursement Department
Qualifications
RN - MDS Nurse Assessor - Qualifications:
Licensed Professional Nurse (RN licensure preferred) with current state license Prior experience with MDS 3.0 preferred Long-term care experience preferred Excellent verbal and written communication skills Self-motivated, highly organized, and dedicated Willingness to train the right candidate
