What will you do: Formulate custom care plans from assessment and clinical information demonstrating the application of clinical guidelines including standards of care. Establish specific, measurable, attainable, relevant and time specific nursing goals to guide the care planning process and promote self-reliance. Assist patients in establishing, modifying, or reinforcing a social support network, thereby promoting self-reliance. Identify and align resources that impact high-risk care. Coordinate services on behalf of the high-risk patients. Work in collaboration with the Healthcare team to track and review data for research projects as well as resource utilization related to relevant specialty and make recommendations as needed. Qualifications Required Education/Experience/Specialized Skills: BSN CA RN License At least three years recent clinical nursing experience in a variety of clinical settings Required Certification/Registration: BLS through AHA Two years experience managing care of relevant transplant patient population Job : RN Primary Location : Central San Diego County-LA JOLLA-SCRIPPS GREEN HOSPITAL Work Locations : SCRIPPS GREEN HOSPITAL 10666 N.
