Admissions Nurse (Monday-Friday PM Shift)

Summary:

The Admissions Nurse is available to perform professional nursing care activities with persons in the admissions process. Role functions are governed by the Wisconsin Nurse Practice Act, the Wisconsin Administrative Code, professional standards for nursing practice, and the corresponding policies and procedures of Rogers Memorial Hospital (RMH). The Admissions Nurse seeks consultation with other members of the health team as the patient's condition and treatment goals warrant.

Job Duties & Responsibilities:

  • Complete and document initial screenings and assessments
  • Coordinate the admission process by collaborating with referral sources, physicians, and other staff to ensure efficient and timely patient entry.
  • Obtain, review, and verify physician orders (including but not limited to admission orders) as needed.
  • Obtain, review, and verify admitting patient's medication history per protocol.
  • Document patient information, assessments, and interventions accurately and in compliance with facility policies and regulatory standards.
  • Complete and ensure completeness of all admission-related documentation and ensure submission within required timeframes.
  • Orient patients and their families (if applicable) to the hospital's policies, procedures, and treatment environment.
  • Provide education to patients and families (if applicable) about treatment plans, medications, and patient rights.
  • Monitor and report any changes in patients' conditions during the admission process to the attending physician promptly.
  • Communicate and collaborate effectively with multidisciplinary teams, including social workers, therapists, medical staff, and psychiatric providers, to ensure comprehensive patient care.
  • Act as a liaison between the hospital, referral sources, and patient support systems to ensure a smooth transition into care.
  • Adhere to hospital policies, procedures, and behavioral health regulations to maintain patient safety and confidentiality.
  • Maintain a safe and therapeutic environment by monitoring patient behavior and implementing de-escalation techniques as needed.
  • Act as an advocate for patients.
  • Participate in quality improvement initiatives and training programs to enhance the admission process and patient care delivery.
  • Perform other related duties as assigned to support patient care and the nursing department

Education/Training Requirements:

  • Associate degree, in nursing, required. Registered Nurse licensed by Wisconsin State Board of Nursing. License must be in good standing with the Wisconsin State Board of Nursing.
  • American Health Association Healthcare Provider CPR certification or American Red Cross Professional Rescue is required within thirty (30) days of date of hire. Annual re-certification is required.
  • Formal training in management of the aggressive patient is required within sixty (60) days of date of hire. Annual re-certification is required.
  • Previous psychiatric experience with children, adolescent and adults is preferred.
