Position Description:

In the role of Social Worker I for the Cancer Center Psycho-Oncology Program you will provide assessment, outpatient behavioral health care, case management, and community referrals to patients within the Psycho-Oncology Program.

Primary Functions

  • Responsible for the management of patients including, and mostly, face-to-face and virtual psychotherapy focused on cancer-related mental health care needs
  • Assumes responsibility on intake and required screenings
  • Provides patient condition education
  • Provides care coordination in the community to facilitate linkages for newly diagnosed and high-risk patients.
  • Collaborates with inpatient/outpatient staff and providers
  • Provides clinical services for adults within the Psycho-Oncology Program
  • Triage of emergent situations, phone conversations, and the ability to negotiate healthcare and social service systems
  • Complete intake assessments and develop appropriate service plans. Document in an electronical medical record (EPIC) as needed
  • Interface with clinical faculty and staff, including MD/DO's, fellows, residents, medical students, therapists, nurses, interns, and support staff. Provide diagnostic consultation; maintain and participate in Quality Assurance activities.
  • Complete documentation requirements for all services provided and participate in clinical supervision/consultation in compliance with all county, state, and federal rules.

Position Requirements:

Knowledge - Skills - Abilities

Psychotherapy

Knowledge of biology, clinical judgment, medicine, cultural awareness, and consulting.

Medical documentation, interpersonal, practice-based learning and improvement, empathy, and decision-making skills.

Specifications

Appropriate experience may be substituted on equivalent basis

Minimum Requirements:

  • Bachelor's degree
  • 3 years Experience
  • Wisconsin provider status or certification

Preferred Qualifications

  • Minimum 3,000 hours post-Bachelor's supervised experience
