The Nursing Coordinator has primary responsibility for scheduling new nursing and ongoing visits and distributing work flow in the department to assure that both new nursing visits and ongoing nursing visits are completed timely and within start of care and patient time expectations.

Schedule:
Monday - Friday
8:30am - 5:00pm
will change when branch goes to new workflow and include a Saturday rotation

Schedules initial Amerita nursing visits for new patients
• Schedules initial and ongoing SUB nursing visits
• Provides requested patient information to SUB to assure appropriate patient care.
• Approves submitted nursing visit records within the electronic documentation platform.
• Provides feedback to Amerita and SUB nurses if additional information is required on the nursing visit record, or if there is missing or inaccurate information on the submitted records.
• Submits Plans of Treatment (POT) to the prescriber to obtain prescriber signatures.
• Tracks these signed POTs and communicates with RN manager when there is difficulty with getting prescriber signatures within 30 days.
• Contacts SUB for outstanding nursing notes and invoices as needed.

• High School Diploma/GED or equivalent
• Basic knowledge and experience in home care preferred
• Good organizational and time management skills
• Excellent verbal and written communication skills
