JOB SUMMARY:

The social worker evaluates a patient's social, emotional, environmental, financial, and support needs. Assesses, plans and acts in response to the psychosocial needs of patients/families. Intervenes in crisis situations to assist patients and serves as a behavioral health resource. Facilitates decision making on behalf of patients and families in complex cases. Collaborates with care manager in assessment and coordination of discharge planning. Provides interventions designed to address the social drivers of health, with focus on the promotion of health, the prevention of disease, and addressing barriers to access to healthcare. A master's prepared social worker will provide mentorship of social work interns during prescribed clinical rotations as applicable.

JOB REQUIREMENTS:

* Licensed Social Worker in TN Required

* Master's in Social Work Required

* 1 Year of Social Services Experience in Health Care Required
