One Robert Wood Johnson Place, New Brunswick, NJ 08901 The Pediatric Palliative Care Social Worker is an integral member of the healthcare team dedicated to providing comprehensive psychosocial support to children with life-limiting illnesses and their families, The social worker collaborates closely with medical providers, nurses, and other disciplines to optimize the child's quality of life, address emotional and practical needs, and facilitate end-of-life care planning, This position encompasses a wide range of responsibilities including psychosocial assessments, counseling, care coordination, and community resource utilization, The social worker will play a pivotal role in empowering families to cope with the challenges of their child's illness, make informed decisions, and access appropriate support services, Qualifications Master's degree in Social Work MSW from an accredited school of social work Licensed Social Worker LSW in the state of New Jersey Three or more years of experience in an acute adult or pediatric setting is preferred Palliative Care certification preferred Essential Functions Conduct comprehensive psychosocial assessments of pediatric patients and their families to identify emotional, social, and practical needs, Provide individual, family, and group counseling to address grief, loss, anxiety, and other emotional challenges, Collaborate with the healthcare team to develop and implement individualized care plans that address the physical, emotional, and spiritual needs of the child and family, Coordinate discharge planning, including identifying and connecting families with appropriate community resources and support services, Advocate for the child and family s needs and rights within the healthcare system, Educate patients, families, and healthcare providers about pediatric palliative care, end-of-life care, and bereavement, Demonstrate a deep understanding of child development and the unique challenges faced by children with life-limiting illnesses and their families, Provide crisis intervention and support as needed, Maintain accurate and comprehensive documentation of patient and family interactions, Scheduling Requirements This is a full-time position 40 hours/week, RWJBarnabas Health is an Equal Opportunity Employer |
