LPN - Case Manager
Salary Pay: $25.00 - $34.00 / hour Overview Elderwood is in growth mode and currently looking for a dedicated and compassionate LPN Case Manager to join our team! In this role, you will assist the Director of Nursing with on call coverage every other weekend and work 11am-7pm during the week. Ideal candidate will have experience with Assisted Living regulations and memory care, along with prior supervisory experience. Case Managers assist with ensuring the health and well-being of our residents by being responsible for the admission of residents; providing case management services for residents; and serving as the designated Discharge Planner for the facility. We're always mindful of personal needs, while seamlessly coordinating important health services. Responsibilities Case Manager/ Licensed Practical Nurse (LPN):
Supervise/Administer and/or assist with medications to assigned residents; monitor monthly residents participating in self-medication program. Qualifications Resident Care Supervisor / Licensed Practical Nurse (LPN):
