Working closely with Heart and Vascular Center (HVC) leadership, the individual will be responsible for coordinating and providing care to structural heart, cardiac and thoracic surgery patients. Responsible for closely working with advanced practice providers and physicians to create value for the Structural Heart and Cardiac Surgery programs. Focuses on patient care, operational and quality metrics, tracks performance progress, and implements improvement initiatives to achieve top HVC performance. EEO/AA/Disability/Veteran.


Responsibilities

Develops the overall plan of care/guidelines and communicates plan to members of the patient care team.

Reviews clinical records to obtain demographic and financial information and assesses physiological
needs, documents appropriate plan of care in the patient care record.

Effectively develops and coordinates individualized plans of care with physician , patient, family and other members of the health care team as evidenced by documentation in the medical record.

Effectively plans for patient transfer or discharge and coordinates resources to provide direct follow up care as evidenced by feedback and supervisory observation.

Coordinates the development of multidisciplinary patient care conferences.

Participates in family meetings, daily rounds and all appropriate meetings to discuss patient?s plan of
care and potential issues.

Provides direct and indirect care to patients and families.

Acts as liaison between patients, families, hospital staff and community agencies to promote
communication and facilitate admission and discharge planning.

Assess psychosocial needs of patients and families and provides counseling and/or referrals as
evidenced by documentation in patient care record.

Communicates plan of care from pre-admission through post discharge with care team, patient and
family members.

Collaborates with team members in coordinating patient/family educational needs and developing
teaching plans as evidenced by documentation in the medical record, and feedback from patient and
family.

Facilitates the patient's progress through discharge by monitoring effective utilization of hospital
services as evidenced by review of length of stay.

Utilizes relevant research findings and quality outcomes to support and advance nursing practice and improve
patient outcomes.

Integrates relevant research findings into practice as evidenced by observation and feedback.

Assists staff in the collection of research data as appropriate as evidenced by feedback from staff.

Reviews NCDR TAVR Registry and/or STS Registry quarterly reports and monthly outlier reports.
Working in conjunction with HVC leadership and departmental resources , identifies areas of opportunity for improvement. Develops qualitative and quantitative analysis to support recommendations.

Facilitates multidisciplinary team to regularly review data and initiate change to achieve top performing benchmarks

Collaborates with YNHHS HVC colleagues to share best practices.

Attends educational seminars to maintain and meet expectations set forth by Hospital and departmental standards.

Maintains knowledge of trends and developments in the field of Heart and Vascular Services.

Attends and participates in in in-service meetings and other designated training events that will enhance skills on a regular basis documented by attendance at training seminars.

Demonstrates and maintains current knowledge and skill in providing appropriate care for patients as
observed by physician team.

Performs other relevant duties as required.

Performs daily in-hospital rounds and weekly meetings with mid-level practitioners to follow inpatients
through their hospital stay.

Conducts post op follow-up with patients including medication reconciliation.

Coordinates care with visiting nurse/home care services and ancillary care providers.
Follows through with patient referrals and maintains open communication with referring physicians for
each patient referral.

Provides pre-op and post-op education and care for Structural Heart, Cardiac and thoracic surgery
patients.

Assists with scheduling patients, conducting and managing clinic.

Ensures adequate stocking of supplies for examination rooms.

Ensures all regulatory and compliance measures are met within departments.

Provides direct supervision to staff in the Cardiac Surgery and/or Structural Heart Programs.

Performs evaluation of staff in department that directly report to Nurse Coordinator.

Coordinates schedules and operations of department staff.

Sets up regular staff meetings with both sections (Cardiac Surgery and Structural Heart).

Ensure staff mandatories and training are completed and up to date.

Coordinates recruitment of new employees.

Works with advance practice providers and physicians to ensure operational needs are being met.


Qualifications

EDUCATION (number of years and type required to perform the position duties):
Bachelor's Degree in Nursing is required. Master's Degree strongly preferred.

EXPERIENCE (number of years and type required to meet an acceptable level of performance):
Minimum of 2 years of relevant clinical experience in the area of specialty. Ideal candidate would have three to five years of professional work experience. Experience in data analysis, reporting and effective presentation required . Proficient in the use of the Microsoft Office Suite, including Excel and PowerPoint.

SPECIAL SKILLS: Excellent verbal and written communication skills. Demonstrated ability to navigate across departments and delivery networks to achieve results. Demonstrated ability to successfully resolve complex problems across multiple departments and/or delivery networks. Clinical credibility with physicians, nurses and other members of the healthcare team. Ability to quickly process complex problems, identify key components, and develop plans for resolution . Effective interactions with all health care team members and the ability to elicit their cooperation. Demonstrated skills in clinical management of specific patient populations including working with and providing support of primary nursing. Ability to provide clinical financial education to colleagues, patients, and families. Critical thinking, negotiation and priority setting of highest importance in this role. Intermediate to advanced computer skills including Word, PowerPoint, Excel.

Ensures that patients are progressing along evidence based paths in accordance with established clinical and financial outcomes; intervenes to correct delays and has the authority to cross departmental lines. Accountable for ensuring high quality care that is coordinated, efficient, and efficacious and supports desired clinical and financial outcomes. Development, implementation, and monitoring for Registry metrics to monitor performance for HVC. Models patient service standards and demonstrates value for all people in the work environment; adhering to the

Works with individual and groups of care team members to help them understand opportunities to change patterns of practice and clinical resource management.

LICENSURE/CERTIFICATION:
Current RN licensure in the State of Connecticut.
