Senior Social Worker

Shift Details:

Varied days (8 AM - 4 PM), including every other weekend and holidays as needed

Job Description

Provides case and group social work services to patients and/or their families during patient's Hospital stay. Provides guidance and direction to social workers, related paraprofessional staff, social work interns and volunteers.

Job Responsibility

• * Provides direct service to patients and their families using a variety of social work modalities in order to help clients maximize their functioning. Travel to patients' homes, community resources, community meetings may be required.
• * Advocates on behalf of the client to ensure maximum access to health care.
• * Maintains appropriate professional patient care documentation as defined by hospital policies and procedures.
• * Functions in a consultative and educative capacity with all pertinent individuals or agencies that impact on patients.
• * Prepares materials for supervisory conferences and appropriate staff development programs.
• * Provides guidance and direction to social workers, related paraprofessional staff, social work interns and volunteers.
• * Assists in the formulation and development of Departmental and interdepartmental programs, policies and procedures.
• Contributes to the assessment of social health needs in the hospital and community. Participates in the development of effective health and welfare services/resources.
• May participate in appropriate research, staff development programs, and community liaison activities.
• Performs related duties, as required.

Job Qualification

• Master's Degree from an accredited school of Social Work, required.
• LMSW, required. LCSW, preferred.
• Minimum of three (3) years post-Master's related clinical work experience, required.
