Supported Employment Associate -Millcreek of Poplarville; Part-Time $10.00/hr

MINIMUM REQUIREMENTS:
1.Must be at least 21 years of age.
2.Must have a high school diploma or GED.
3.Must have a valid driver's license.
4.Must have reliable transportation and meet applicable requirements for Millcreek's Motor Vehicle Safety Program.
5.Must be willing to work flexible schedule.
6.Must complete HCBS DCW training program.

ESSENTIAL JOB FUNCTIONS:



  1. Attend orientation/training as scheduled.
  2. Maintain reports and daily logs and submit to Supervisor in the assigned timeframe.
  3. Notify Supervisor in advance if unable to complete assigned shifts.
  4. Act as advocate/liaison between individual and co-workers and/or individual and supervisor.
  5. Monitor relationships (individual/co-workers, individual/employer) to ensure that positive and supportive communication occurs.
  6. Review employer's policies, expectations, and, requirements with the individual.
  7. Assist individual to participate in job explorations and other job-related activities as needed.
  8. Orient individuals to work site, i.e., time card, restrooms, break areas, rules, etc.
  9. Train individual regarding all safety rules on the job site.
  10. Model appropriate work behaviors to enhance/reinforce individual employment skills.
  11. Perform work tasks not performed by individual to the employer's specifications in order to meet the obligation to the employer.
  12. Provide on-going job modification to solve individual's skill-related problems.
  13. Provide on-going assessment and recording of individual's performance, productivity, and behavior.
  14. Provide transportation for the individual to and from the worksite.
  15. Maintain liability insurance on personal vehicle in order to transport individuals to and from the worksite.
  16. Notify Supervisor of any accident or moving violation within 24 hours of its occurrence.
  17. Adhere to all Millcreek and HCBS ID/DD waiver program policies and procedures.
  18. Follow all safety policies and adhere to all workers' compensation program guidelines.
  19. Other duties as assigned.
