We are seeking a compassionate, full-time, 40-hour Community Health Worker (CHW) to join our APP team! This hybrid role offers a flexible schedule, ideally Monday - Friday, 8 am - 5 pm, with 4 days on-site and 1 day remote. While primarily supporting our Dedham office, the CHW may need to commute to another APP practice within a reasonable distance to provide support.
The CHW will work closely with patients, families, and community resource agencies to promote and maintain good health. This includes providing education, support, and outreach services to help community members achieve their health goals. Areas of focus may include mental health, medication management, nutrition, healthcare system navigation, resource access (food & housing), substance use, and other health behaviors. The CHW works under the direction of a healthcare professional to deliver preventive care and health education to the community. Essential Functions: *Conduct community outreach to engage individuals and assess their health needs. *Provide individual and group health education and coaching to community members. *Assist community members in navigating the healthcare system and refer them to appropriate services. *Perform basic health screenings and assessments to identify health risks and provide suitable interventions. *Help develop health promotion programs and activities that address community health needs. *Collect data and maintain accurate records of outreach activities and interventions. *Attend meetings and training related to community health promotion and education. Due to the hybrid nature of the role, we ensure that our employees receive the required technology and training to be proficient and independently productive in all job responsibilities regardless of work location. For the remote workdays, employees are responsible for designating a workspace within the remote work location that is private, safe, ergonomic, and free from distractions for all hours worked. Required: High School Diploma or Equivalent Preferred education/ experience
