The care coordinator serves as a patient advocate and point of contact when a recommendation for care is received. They assist by ensuring successful navigation from program recommendation to admission. The patient coordinator works closely with many other disciplines, including admissions staff, site specific operations, leaders, and medical staff to ensure patients have the highest level of support and care from recommendation for services to treatment.

Job Duties and Responsibilities:

The Care Coordinator 1 position is a key role within Rogers Behavioral Health, responsible for managing inquiries, conducting screenings, coordinating admissions, and ensuring a seamless continuum of care for patients. This position involves not only screening potential patients and facilitating their admission but also maintaining communication and collaboration across various departments to ensure the timely and efficient delivery of care.

Pre-Admission Process:

  • Handle incoming inquiries, conduct screenings, and explain steps to access care.
  • Describe Rogers' levels of care, admission criteria, and services to potential patients and their families.
  • Assist with patient, family member, and guardian inquiries or questions regarding the admission process.
  • Gather and verify demographic and insurance details, including pharmacy insurance information, collaborating with Patient Financial Services (PFS).
  • Coordinate and schedule admissions appointments, ensuring the completion of necessary paperwork.
  • Review clinical information with clinicians to address safety concerns and support admission recommendations.
  • Ensure timely follow-up with patients and families, and maintain an open line of communication.

Program Coordination & Patient Experience:

  • Relay recommendations of appropriate programs and services to patients and families.
  • Provide ongoing support to patients on the waiting list, performing weekly check-ins to ensure they are informed and supported.
  • Develop and maintain strong relationships with interdisciplinary team members to ensure open communication and collaboration regarding patient care. Key teams include Inpatient, Outpatient, Residential Admissions, Utilization Review, Business Development, Patient Financial Services, and Health Information Management.

Documentation & Compliance:

  • Accurately enter inquiries, referral details, clinical information, and program-related data into the appropriate systems.
  • Adhere to policies and regulatory standards, including HIPAA, EMTALA, and Joint Commission requirements.
  • Participate in training and development to stay informed on regulatory standards, processes, and best practices.

Support and Collaboration:

  • Collaborate with internal teams, external facilities, and other resources to ensure seamless access to care.
  • Connect patients or their representatives to external resources when needed, and ensure continuous support throughout their care journey.
  • Maintain ongoing communication with the patient and their family to address any concerns and ensure the best possible experience.

Other Responsibilities:

  • Provide support for on-call, weekend, and holiday schedules as required.
  • Complete additional tasks as assigned by leadership to support the goals of the department and organization.
  • Uphold organizational policies, department objectives, and compliance with all relevant regulatory standards.
Schedule: Either option: Sunday-Wednesday 9:30A-8PM OR Sunday-Thursday 11:30A-8P

Additional Job Description:

Education/Training Requirements:

  • Bachelor's degree preferred. High school diploma required. Two years of customer service and relevant experience required; preferably in a healthcare setting.
  • Experience with computers and demonstrated proficiency with software platforms and EMRs.
  • Knowledge of medical/psychiatric/chemical dependency diagnosis, treatment process, managed care, Joint Commission, preferred.
  • Oconomowoc Only: Valid driver license preferred and must be granted insurable status by the Rogers insurance policy.
