Location: Blackburn Center (12121 E. Burnside Street, Portland, 97216)

Schedule: Monday - Friday (8:00am - 5:00pm)

Compensation ($$$): $53,913.60 - $75,483.20

MINIMUM QUALIFICATIONS (REQUIRED)

    1. Required: Minimum one (1) year of supervisory experience.
    2. Required: Minimum one (1) year experience in community health center or primary care.



    PREFERRED QUALIFICATIONS (NOT REQUIRED, BUT GREAT TO HAVE)



    1. Bi-lingual (e.g. Spanish-speaking).
    2. Two (2) or more years of supervisory experience.
    3. Bachelor's Degree or higher in health or social service related field of study.



    SUMMARY:

    The Blackburn Center is an innovative program that will offer integrated housing, health care, and employment services with the long-term goal of eliminating homelessness. We seek to change how these services are delivered to all people, especially vulnerable populations.

    This position will work as a member of a multi-disciplinary team at the Blackburn Center to deliver comprehensive, person-centered, integrated services to the Portland Eastside community. The Care Team Manager is a leader of the patient-centered care team, who supervises support staff and helps facilitate clinical services within each care team. The CTM is responsible for delegation and supervision of the work of the primary care home, ensuring care that is accountable, comprehensive, proactive, well-coordinated and patient-centered, addressing medical, mental health, addiction, and behavior needs.

    ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:

    General:



    1. Work as a member of the Blackburn Center multi-disciplinary team to deliver comprehensive, client centered and trauma informed integrated services.
    2. Actively participate in the integration of services at the Blackburn Center and identify new opportunities to improve collaboration between and within service and care teams while carefully following information-sharing guidelines.
    3. Proactively identify opportunities for improvement within Blackburn Center workflows and logistics. Participate in review of Blackburn Key Performance Indicators; review and contribute to continuous improvement opportunities and plans.
    4. Build understanding of and appreciation for diversity among Blackburn Center clients and support their connection to culturally responsive and culturally specific services as requested.


    Clinical Care:



    1. Responsible for patient related documentation, as directed and in collaboration with Lead Care Team Manager RN and provider.
    2. Supervises support staff to ensure that needs of the patients are met, in conformance with established policies and procedures.
    3. Coordinates with provider, pharmacy and patients for medication adherence and support as directed.
    4. Coordinates clinical care and referrals within CCC programs and with community partners.
    5. Coordinates multi-disciplinary patient care conferences for complex patients.



    Team Leadership:



    1. Guides care team in setting and achieving performance goals.
    2. Participates in regular care team manager meeting.
    3. Plans and facilitates monthly "clinical team" meeting.
    4. Supervises and/or delegates completion of staff scheduling; including rotating on-call duties.
    5. Supervises and/or delegates time card management and time off requests.
    6. Supervises completion of weekly team/clinic/lab tasks.
    7. Supervises call management and documentation quality within care team.
    8. Supervises and/or delegates outreach for proactive preventive care and panel management.
    9. Supports clinic wide staffing needs.



    Quality and Compliance:



    1. Directs innovation in care team and clinic practice through continued quality improvement initiatives.
    2. Adheres to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.
    3. Attends all mandatory CCC trainings in a timely manner.
    4. Performs other duties as assigned.



    SKILLS AND ABILITIES:



    1. Ability to consider the impacts and outcomes for underserved communities during decision-making processes.
    2. Ability to consider impacts of systems of oppression, structural racism, and individual bias on client outcomes.
    3. Experience in a medical or health care setting.
    4. Experience with underserved populations and homelessness preferable.
    5. Interest in public health, primary and preventive nursing/medicine.
    6. Understanding of substance use and mental health issues.
    7. Working knowledge of principles of harm reduction and trauma informed care.
    8. Excellent oral and written communication skills.
    9. Computer proficiency.
    10. Demonstrated ability to prioritize.
    11. Ability to successfully interact with a wide variety of people including the patients and co-workers.
    12. Ability to manage time and meet deadlines as needed.
