POSITION SUMMARY


CLUES' Certified Community Behavioral Health Clinic (CCBHC), Abriendo Caminos, provides timely access to supportive, high-quality, culturally-responsive and integrated Behavioral Health services and pathways to wellness for the Latinx Community, regardless of insurance status or ability to pay. The CCBHC model provides CLUES with a model for sustainable Behavioral Health service provision, which translates to increased access to consistent care for wellness in the Latinx community.

CLUES' Manager of Mental Health Case Management is responsible for ensuring the quality of services provided by the program and achieving program outcomes; supervising, training and developing program staff; and acting as a liaison for the program and department with funders and external entities. Responsible, with the department leader, for finding opportunities and reaching agreements to expand services and support the financial viability of the department.

Estimated Salary/Pay: $81,500.00 - $96,000.00 (depending on experience & qualifications).

ESSENTIAL DUTIES AND RESPONSIBILITIES

Mental Health Case Management Program Supervision/Management

  • Provide leadership and direction to the staff in the program/department to promote an optimal level of service by consistent high-quality oversight. Create a supportive, client-centered environment to effectively respond to the needs of clients and their families.
  • Verify that program staff are providing services in a culturally responsive and linguistically appropriate manner.
  • Provide clinical supervision and support the professional development of Children's and Adult Mental Health Targeted Case Management (CMH-TCM and AMH-TCM) providers.
  • Collaborate with Behavioral Health leadership and staff to provide integrated care within the CCBHC model.
  • Manage programmatic and budgetary aspects of this program.
  • Promote behavioral health services both internally within CLUES and externally within the community, while also assisting staff in areas including but not limited to quality, outcome-based design, service relevance, delivery, effectiveness, and evaluation, all in alignment with CLUES mission.



Program Development, Financial Management & Compliance

  • Supervise the daily operations of and provide clinical supervision for Children's and Adult Mental Health Case Management Program, providing bilingual and culturally responsive case management services to children who meet eligibility criteria for "Severe Emotional Disturbance (SED)", and adults who meet eligibility criteria for "Serious and Persistent Mental Illness (SPMI)", according to Minnesota statutes, who have been referred by Ramsey, Hennepin, or other counties. Conduct Comprehensive Evaluations for case management referrals as needed.
  • Coordinate billing for both Children's and Adult Mental Health Case Management programs with administrative support staff.
  • Represent CLUES in Mental Health Case Management program-related meetings. Serve as a liaison with Ramsey and Hennepin Counties for these programs and the department leader (for example, coordinate and attend monthly Ramsey County Children's Mental Health Case Management Supervisor meetings). This would also apply to other counties if necessary.
  • Ensure staff work with families to develop client treatment/case plans and crisis plans as well as coordinate and facilitate access to needed community and county services. Support staff in building professional relationships with individuals, children, and families while helping them navigate the mental health system. Support staff in building effective professional collaborations with community providers.
  • Guide staff in appropriately managing crisis response situations with clients.
  • Conduct direct client case management services as needed to provide coverage for the team during absences such as vacations, leaves, vacancies, etc., ensuring continuity of care for clients as well as continuity of services to meet program goals and outcomes. In rare circumstances when providing direct client services, transport clients to meetings, appointments, and court dates as needed.
  • Ensure progress toward and achievement of contract and program goals for accurate and timely reporting. Manage and support staff to achieve program goals (including productivity targets), complete documentation, maintain records, and report outcomes and activities according to funding and agency requirements. Ensure clinical documentation is kept up to date and in audit-ready condition.
  • Collaboratively work with department leaders with a focus on continuously improving program effectiveness, efficiency and financial viability. Support the department leaders in setting program goals and objectives, and planning strategies for meeting goals. Assist department leaders in the delivery of contracts outcomes and relationships with contractor stakeholders.
  • Assist the Director of Behavioral Health Services to effectively manage department finances, including program budget and resource allocations relative to financial targets and department goals.
  • Maintain processes to collect relevant data to track and report upon outcomes in a timely manner according to grant/ contract requirements and department goals.
  • Understand and adhere to all laws, regulations, rules and policies applicable to services offered by the Behavioral Health department. Ensure compliance with funder/contract requirements and comply with auditing requirements.
  • Maintain client confidentiality in accordance with HIPAA, agency policies and other applicable laws, regulations and ethical standards while ensuring staff within the Behavioral Health department do the same.



Personnel Management and Development

  • Collaborate with supervisor and human resources department to plan, implement, and manage staffing to maximize effectiveness. Recommend staffing structure and personnel decisions including hiring, promotion, disciplinary action and termination to supervisor and HR.
  • Effectively supervise department staff, including: hiring, providing guidance and direction, training, evaluating performance, initiating corrective action or disciplinary action and other personnel actions.
  • Provide direction, training, and coaching to Children's and Adult Mental Health Case Management staff, including new staff on-the-job training and department orientation, etc. Provide on-going training and support regarding current case management regulations and processes with Ramsey, Hennepin and other counties as necessary. Provide constructive feedback as well as positive recognition.
  • Ensure staff have adequate training and necessary certifications or licensure when required, such as training programs, webinars, and other professional development opportunities to meet current position requirements and expectations. Mentor and encourage staff professional development.
  • Monitor individual productivity goals for staff based on department goals. Provide clear and timely communication to staff regarding expectations and delegated tasks. Maintain regular and timely documentation of staff performance. Meet regularly with department staff to oversee progress toward individual and department goals.
  • Develop a well-functioning team, fostering staff empowerment. Nurture a safe, dynamic and team-based working environment. Promote team building through group activities and staff recognition.



General Duties

  • Promote the mission, operating values, and agency services of CLUES within the community.
  • Partner with other leaders to effectively deliver integrated client services. Ensure that Behavioral Health staff collaborate proactively with other programs in CLUES to achieve service integration and provide comprehensive array of services to clients.
  • Attend program-related trainings to develop personal knowledge and skills relevant to increasing program and project effectiveness as well as training staff.
  • Attend and facilitate case conference sessions and other client or staff meetings.
  • Cultivate working relationships with potential service collaborators and/or partners. Provide consultations and presentations to other professionals and organizations as assigned.
  • Demonstrate appropriate leadership and workplace behaviors, setting an example for staff and clients to emulate.
  • Maintain open communication with supervisor and staff, soliciting feedback as appropriate.
  • Provide services in multiple locations as directed.
  • Carry out all other duties as assigned by supervisor.

SUPERVISORY RELATIONSHIPS


  • This position provides clinical and administrative supervision for the Children's and Adult Mental Health Targeted Case Managers, and may supervise other program staff, interns and/or volunteers as assigned.


QUALIFICATION REQUIREMENTS

Education, Experience & Key Abilities

  • Master's degree in social work, counseling, psychology, Marriage and Family Therapy, or related field from an accredited university required.
  • 5 years of experience in the field and supervisory experience in Adult and/or Children's MH-TCM preferred.
  • Independent licensure or Minnesota recognized clinical licensure in marriage and family therapy (LMFT), social work (LICSW), counseling (LPCC), psychology (LP), or other appropriate licensure required.
  • Board approved for clinical supervision within discipline preferred (e.g., Board certified clinical supervisor as LMFT, LICSW, LPCC, or LP).
  • Must have a deep understanding of Latinx cultures, values, and traditions, and demonstrate cultural responsiveness in every aspect of the role, including communication with colleagues, staff, clients, community members; creating forms and documents; hiring and training staff; and in clinical supervision and consultation.
  • Bilingual in Spanish and English and cultural responsiveness and awareness required.
  • Experience supervising and leading successful team(s) preferred.
  • Experience managing programmatic and financial aspects of social service program preferred.
  • Must be organized, have the ability to work under pressure and stress, have attention to detail and the ability to multi-task in a fast-paced environment.
  • Self-motivated and proactive problem solver.
  • Knowledgeable of and experienced in accessing community resources that can serve participant needs preferred.



Language Skills

  • Effective communication in English and Spanish, both verbally and in writing, required.
  • Ability to read, analyze, and interpret written materials, including reports, contracts, and proposals.
  • Ability to effectively present ideas and information in an organized and effective manner, including writing reports.
  • Ability to maintain accurate documentation and respond appropriately to questions from clients, co-workers, management and external parties.



Transportation

  • Must have access to use a reliable motor vehicle for independent transportation, valid driver's license, proof of automotive insurance, and maintain a good driving record. Must submit to and pass annual recertification of compliance with CLUES Automobile and Transportation Policy and driving record check.


Computer Skills

  • Proficient working knowledge of email software, the Internet, databases, and common Windows-based programs, including Microsoft Office Word, Excel and PowerPoint required.
  • Must be willing to attend training to advance technical skills and maintain appropriate skill levels to perform job duties.

Background check

  • Must pass a criminal background check, driving record check, and MS-604.20 background check. As a condition of continued employment, staff person may be required to submit to periodic re-verification of these background checks.
