UMR offers tailored programs designed to enhance clinical and behavioral outcomes. In this role you will aid in changing our members' behavior regarding their high risk for development of chronic diseases. This metrics-driven Health Coach role requires telephonic outreach to members that have been pre-identified. Telephonic interactions with members are structured around behavior change theory and seek to assist individuals make healthy lifestyle changes. Health education and self-advocacy are also key tools in this role.

Primary Responsibilities:



  • Demonstrate an ability to deliver effective telephonic coaching in a manner that promotes behavior change, including assisting clients in creating Health Vision and SMART goals
  • Utilizes motivational interviewing techniques for assisting members to move through stages of behavioral change
  • Utilizes established standards of care to identify and address members' gaps in care
  • Document clinical information completely, accurately and in a timely manner following established documentation standards
  • Participate in weekly meetings and monthly in-service training
  • Appropriately adapt telephonic coaching and education methods to accommodate the unique physical, psychosocial, cultural, age-specific needs of each member served
  • Demonstrate established customer service behaviors and standards
  • Contribute to program quality and enhancement through critical program support activities such as development of member presentations, health education materials and/or module reviews
  • Support Inbound phone lines as needed
  • Outbound recruitment as needed

Required Qualifications:



  • Bachelor's degree in nursing, health education, nutrition, exercise physiology or closely related field
  • Certification in a nationally recognized coaching program preferred or enrollment in a nationally recognized coaching program within 12 months of hire and achievement of certification within 3 months of the completion of the program
  • 2+ years of experience required in health, nutrition or exercise education, health promotion or worksite wellness
  • 1+ years experience with electronic documentation
  • Proficient in PC-based word processing and database documentation (Word, Excel, Internet, Outlook) and the ability to type and talk at the same time
  • Demonstrated insight into human behavior and have the ability to communicate and interact with individuals at varying levels of skill and education preparation
  • Basic knowledge of disease prevention and basic understanding of exercise principles, nutrition, and weight management

Preferred Qualifications:



  • Telecommuter experience
  • Ability to navigate multiple computer programs simultaneously
  • Ability to speak Spanish

Residents Only: The hourly range for this role is $23.22 to $45.43 per hour. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
