Advanced Practice Provider-Cardiology

The Cardiology Advanced Practice Provider provides medical services to assigned patients under the supervision of a licensed physician within the supervising physician's scope of practice. They're responsible to provide acute, chronic and preventative health care in collaboration with the cardiologist in both outpatient and inpatient settings. The APP manages patietns by assessing, diagnosing, coordinating care, educating, and documenting pertinent information with the supervising physician to meet patient care, clinical, hospital and national guidelines. They serve as a resource and educator for patients, family members, and fellow staff. Services include but are not limited to: clinical evaluation of patients; ordering, conducting and interpreting diagnostic and laboratory tests; prescription of pharmacologic agents and nonpharmacologic therapies; and teaching and counseling individuals and groups.

1. Works autonomously and in collaboration with the supervising physician to assess and diagnose patients and formulate treatment plans that include pharmacological, diagnostic, and therapeutic procedures and treatments.
2. Works autonomously and in collaboration with the supervising physician to assess and diagnose patients and formulate treatment plans as below:
   
   
   
   1. Diagnoses patients by synthesizing and analyzing the collected data; formulates a differential diagnosis based on the history, physical exam and diagnostic test results.
   2. Identifies and implements a patient treatment plan which may include the ordering of diagnostic and therapeutic procedures and treatments; selecting appropriate pharmacologic and nonpharmacologic interventions; developing a patient education plan; and making appropriate treatment referrals.
   3. Implements patient care plans based on current care guidelines and clinical specialty expertise; accurately conducts and interprets diagnostic tests; accurately prescribes pharmacologic and nonpharmacologic therapies; provides relevant patient education and makes referrals to other health care professionals and community agencies as needed.'
   4. Evaluates patient responses to the disease process and therapeutic intervention(s), interprets diagnostic studies within the scope of practice and communicates progress to supervising cardiologist.
   5. Determines the effectiveness of patient treatment plans; documents patient care outcomes; reassesses and modifies treatment plans as necessary to achieve health and medical goals.
   
   
   
   
3. Responsible for evaluating and stabilizing the acutely ill patient including all modalities of ACLS.
4. Responsible for admitting and discharging patients, making daily rounds and providing consultative cardiology services for inpatients.
5. Responsible for managing outpatient clinic patients, both new and established.
6. Responsible for the sharing of information with the interdisciplinary team regarding the patient's progress and care.
7. Diagnose, treat, and manage cardiology-related health problems and chronic conditions according to the scope of practice.
8. Provide short and long-term follow-up care, assess risk factor modification, tolerance of treatment, and manage health care needs of patients.
9. Responsible for accurate and timely documentation in the medical record of all clinical encounters compliant to billing standards.
10. Fostering the development and educational processes for students and colleagues and serve as a role model for other staff.
11. Maintain current and progressive standards through participation in continuing educational programs and review of the current literature in accordance with state licensure and facility requirements.
12. Supervises and interprets tilt table tests and exercise stress testing.
13. Supervises exercise as well as pharmacologic stress testing with and without myocardial perfusion imaging and echocardiography imaging.
14. Interprets/makes clinical decisions regarding cardiac device transmissions including scheduled remote checks, in-clinic device checks, alert transmissions and physiological monitoring transmissions of cardiac devices such as loop recorders, pacemakers and defibrillators. Evaluates for any abnormalities and coordinates a plan of action if required as delegated by and under the guidance of the supervising cardiologist.
15. Responsible for managing their practices' resulting process of any ordered tests/diagnostics with an appropriate plan of care.
16. Responsible for managing anticoagulation (PT/INR) encounters for their practice.

EDUCATION AND EXPERIENCE:

APRN/NP:

Must be a graduate of an accredited school for APRN/NP. Current Nebraska state license to practice medicine as an APRN/NP and Registered Nurse (RN). Active and current certification through either American Nurses Credentialling Center (ANCC) or American Academy of Nurse Practitioners (AANP). Current DEA licensure. Continued CME to satisfy state, licensing board, and stroke certification requirements.

PA-C:

Must be a graduate of an accredited school for Physician Assistant. Current Nebraska state licensure to practice medicine as a Physician Assistant. Active and current certification with the National Commission on Certification of Physician Assistants (NCCPA). Current DEA Licensure. Continued CME to satisfy state, licensing board, and stroke certification requirements.

OTHER CREDENTIALS / CERTIFICATIONS:

Basic Life Support (CPR) certification required. Bryan Health recognizes American Heart Association (for healthcare professionals), American Red Cross (for healthcare professionals) and the Military Training Network.

Advanced Life Support (ACLS) is required.
