JOB SUMMARY

The Mental Health Rehab Specialist/Case Manager III reports to the Program Director and provides specialized services to clients in gaining access to needed medical, social, housing, economic, educational, and other direct services.

ESSENTIAL JOB RESPONSIBILITIES



  1. Will provide mental health services to children, youth and care providers in settings such as client residence, school, community locations, tele-health and clinic sites.
  2. Will provide mental health services aligned with Consumer Wellness Plan (CWP).
  3. Will provide mental health interventions that focus on behavioral improvements such as teaching social skills, coping skills, anger managements skills to help clients achieve treatment plan goals.
  4. Will provide case management services intended to reduce barriers to treatment and provision of needed services through linkage to other community partners.
  5. Will collaborate with community partners as part of mental health activities.
  6. Will complete progress notes documenting service provision in electronic medical record (EMR) in timely manner and in accordance to standards designated by Program Director.
  7. Employees shall follow the reporting requirements for incidents of Child Abuse, Elder Abuse, Adult Abuse, Dependent Adult Abuse and Tarasoff warnings per California reporting laws.
  8. Will manage referrals via telephone, walk-in, fax or email.
  9. Ensure all new referrals are logged and entered into the Avatar Electronic Medical Record (EMR) in a timely manner.
  10. Meet directly with parents/guardians to complete initial admission documentation and enter new episodes in the Avatar EMR.
  11. Will provide crisis evaluation assessments, including the use of de-escalation techniques, develop contingency plans, make recommendations for involuntary hold placement and interface with county psychiatric emergency team staff. Will enter new episodes in the Avatar E.M.R. as part of crisis documentation.
  12. Will provide therapeutic behavioral services (TBS) to consumers, as assigned by Team Leader.
  13. Will provide personal service agreement (PSA) services, as assigned by Team Leader.
  14. Will attend job-related trainings and also participate in a variety of in-service training, seminars, and practicum.
  15. Will attend weekly staff meetings and weekly supervision meetings.
  16. Will drive on Agency business.
  17. Physical presence in the office is required.


ESSENTIAL JOB REQUIREMENTS



  1. Bachelor's degree in social work or counseling in a related field and four (4) years full-time work experience providing direct client services in a mental health, social service, educational or similar environment.
  2. Communicate effectively in written and spoken English.
  3. Ability to communicate effectively in written and spoken Spanish preferred.
  4. Work effectively with diverse ethnic, religious, socioeconomic and professional groups of people.
  5. Ability to operate within Electronic Medical Records (EMR) system.
  6. Ability to operate tele-health platforms.
  7. Intermediate computer skills in Microsoft Word, Excel, Outlook and PowerPoint, Office 365, Teams, Zoom and other communication/meeting platforms.
  8. Possess a valid driver's license and have access to a dependable means of transportation that is properly insured and operated in accordance with all laws. Agency insurer authorization is required.
  9. Ability to pass a criminal background check.
  10. Ability to pass a pre-employment drug screen and TB test.


ESSENTIAL JOB REQUIREMENTS FOR BILINGUAL SKILL



  1. Communicate effectively in written and spoken Spanish.
  2. Required ability to communicate effectively with monolingual Spanish speaking clients and families for all clinical purposes.
  3. Assist with phone calls for Spanish speaking clients
  4. Required to provide language translations for case managers and Physicians on a periodic basis.
  5. Other translation tasks as required.

    • Monolingual: Class 125 ($21.75 - $26.46/hour)
    • Bilingual: Class 130 ($21.13 - $27.81/hour)
