This is a federally and state funded position sponsored by the Texas Child Mental Health Care Consortium (TCMHCC). TCMHCC was created by the 86th Texas Legislature to leverage the expertise and capacity of the health-related institutions of higher education to address urgent mental health challenges and improve the mental health care system in this state in relation to children, adolescents, and perinatal women.

The University of Texas Dell Medical School is a Health Research Institute (HRI) for the TCMHCC. UT Health Austin, Dell Medical School administers the Texas Child Mental Health Clinical (TCMHC) programs to provide mental health services to children, adolescents, and perinatal women. The Nurse Case Coordinator will serve as the primary point of contact for patients served at Dell Medical School, Dell Children's Medical Center, and UT Health Austin and support integration of TCMHC programs, including the Texas Child Health Access Through Telemedicine and the Child and Perinatal Access Networks. TCHATT provides telemedicine or telehealth programs to school districts to help identify and assess the behavioral health needs of children and adolescents and provide access to mental health services. CPAN and PeriPAN provide primary care clinic providers and OBGYNs prompt access to on-call psychiatrists to provide expertise in assessment, treatment, and referrals for pediatric and/or perinatal patients identified as having a mental health challenge.

This position has an end date of 8/31/2025, but may be renewable based upon availability of funding, work performance, and progress toward goals.

Purpose

The Department of Psychiatry and Behavioral Sciences at the Dell Medical School is seeking a highly motivated and compassionate individual to join Dell Medical School as an RN Patient Care Navigator. This position will serve as the primary point of contact throughout the cycle of care for patients served in the Texas Child Mental Health Clinical programs to include collecting vital signs, conducting patient assessments during clinic visits and assisting providers during patient exams.

Responsibilities

  • Reviews clinic schedule and assists with appointment scheduling, including communicating with patients, family members, and clinical care team as appropriate. Assists with new patient referrals by gathering necessary information, including appropriate medical records and other critical data. Liaises with the clinical team for clinical review of referrals. Coordinates and processes patient care forms, collaborating with clinical provider(s) to ensure forms are reviewed, completed, and approved. Identifies and addresses potential barriers to care. Coordinates scheduling of appointments and assists with tracking referrals and treatments.

  • Works collaboratively with the clinical team to complete patient care tasks within electronic health record (EHR) and updates relevant patient information to maintain an accurate and thorough patient record. Reviews patient clinical progress and coordinates communications to referring, consulting, and primary care physicians. Attends multidisciplinary meetings and huddles to convey the patient's situation and background to the team, provide recommendations, and create a comprehensive discharge plan. Communicates with psychiatrists and other team members to ensure continuity of care, both within the programs and external to the university and hospital.

  • Supports and participates in educational and research initiatives in the Texas Child Mental Health Clinical (TCMHC) programs. Identifies and facilitates collaboration and implementation of psychiatry services across TCMHC programs. Participates in continuous quality improvement projects to better serve the patient, family, and healthcare system and improve the quality of service. Participates in critical evaluation of patient care to ensure the program continues to improve and evolve in its care pathways.

  • Serves as a patient advocate, providing direction and support to the patient and family, including educating the patient and family members on the treatment and discharge plan. Conducts transitions of care activities with the patient and family, including, but not limited to: virtual case management for psychoeducation, supports for social determinants of health (SDOH), identification of behavioral health needs, and health system utilization.

  • Conducts consultations with referring providers to identify correct psychiatry service pathway. Creates a plan for care across the continuum, integrating patient/family preferences and values. Communicates effectively with patients, families, and the public to build trusting relationships across a broad range of socioeconomic and cultural backgrounds. Advocates for the patient and family, identifies resources, and works to remove barriers.

  • To enhance the patient experience and reduce delays in care, may support provider teams by assisting with in-basket management, refilling medications, completing prior authorizations, and triaging patients in a timely manner following established protocols.

  • Conducts outreach and provides educational opportunities identified by TCMHC programs leadership. This may include attending community events, tabling at conferences, providing Continuing Medical Education (CME) talks, and visiting behavioral health, primary care, and obstetrics and gynecology (OB-GYN) offices. Assists in the development of departmental protocols, policies, and procedures.

  • Performs other duties as assigned.

Required Qualifications

Registered nurse (RN) with three years of clinical pediatric experience. Experience providing case management in a multidisciplinary care team. Understanding of evidence-based practices in mental health.

Relevant education and experience may be substituted as appropriate.

Preferred Qualifications

Pediatric psychiatry experience. Experience in developing and delivering educational content.

Salary Range

$78,000+ depending on qualifications
