Job Summary

Sobering Center Case Management is a low-barrier, intensive case management service for frequent utilizers of the San Francisco Sobering Center and participants in the Managed Alcohol Program (MAP). The program is based at the Sobering Center and works in collaboration with the Sobering Center and MAP staff to engage clients onsite, in the streets, in shelters, in residential treatment facilities, or wherever clients might find themselves. The incumbent collaborates with community partners such as the Sobering Center nurses and physicians, MAP nurses and physicians, San Francisco Fire Department paramedics, and other community-based non-profits.

The purpose of this position is to engage with high utilizers of the San Francisco Sobering Center and MAP participants.). The incumbent works to decrease the clients' over-reliance and over-utilization of emergency services by linking the client to preventive care including primary care provider, psychiatric care, and specialty medical care. The incumbent participates in care coordination, service initiation, and stabilization of these individuals. Since this population largely has co-occurring substance use disorders, homelessness, and mental health disorders, services must be provided in a culturally appropriate, strength based, and harm reduction approach.

The final salary and offer components are subject to additional approvals based on UC policy.

Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.

The salary range for this position is $29.79 - $38.74 (Hourly Rate).

Department Description

The San Francisco General Hospital (SFGH) is dedicated to providing services that integrate medical, psychiatric, and addiction treatment. SFGH is the largest provider of acute psychiatric care and the only San Francisco provider of psychiatric emergency care.

Required Qualifications

* BA/BS degree; or an equivalent combination of education and experience; and knowledge and abilities essential to the successful performance of the duties assigned to the position.
* Spanish speaking. Able to provide counseling and case management in Spanish.
* One year of work experience in the field of substance use, behavioral health, community outreach, or other human services.
* Demonstrated advanced skills in evidence-based substance abuse counseling.
* Strong interpersonal skills and experience with populations diverse in gender, sexual orientation, and ethnicity.
* Ability to work independently and safely while outreaching to low-income neighborhoods.
* Ability to work cooperatively and communicate effectively with others in multicultural environments. Ability to work in a high stress, high volume area and work well under pressure and with deadlines. Ability to follow oral and written instructions accurately; detail oriented. Able to efficiently prepare written summaries of activities, set priorities and complete reports within required time frames.
* Demonstrated excellent attendance and reliability
* Knowledge of ethical, legal, and confidentiality issues pertaining to substance use.
* Able to work in various environments, including SRO, street, jail, and hospital settings.
* High level of mobility in order to be able to provide outreach at various locations.
* Able to quickly learn, use, and problem-solve electronic health records, Word and Excel documents.
* Able to lift 50lbs and climb stairs
* A preliminary job offer and assignment may be extended pending successful background clearance. Must clear background checks related to federal healthcare programs, before employment and periodically.

Preferred Qualifications

* Community outreach experience with, and experience providing services to, clients with severe substance use disorders.

License/Certification

* Registration or certification with a California approved alcohol and drug counselor certification organization (CCAPP, CAADE, or CADTP).
o If not registered, certified or licensed when applying for the position, the candidate must be eligible to register with one of the entities above.
