This is an entry level clinical pharmacist position. The Home Infusion Clinical Pharmacist I supports the vision, mission, and goals of YNHHS and the Department of Pharmacy and demonstrates competency in all basic clinical pharmacist skills.

Responsibilities

  • 1. Safely and correctly compound sterile medications and infusions for the home, outpatient clinics, skilled nursing facilities and physician/clinic environments.
  • 2. Initiate infusion therapies with successful on-going evaluation of care and completion of required documentation relative to processing therapies, both fiscal and legal.
  • 3. Responsible for understanding of, compliance with and participation in Quality Assurance, Performance Improvement and accreditation guidelines ensuring achievement of excellence in quality patient care.
  • 4. Assist with the referral process including but not limited to: provision of medical information to patients and caregivers relative to home infusion therapy; assistance in the teaching of patients prior to discharge to home; and coordination of deliveries with nursing and support services.
  • 5. Be actively involved in the education and inservicing of licensed health care professionals who perform services relative to the therapies provided by the home infusion center.
  • 6. Maintain working knowledge of all equipment and supplies necessary to administer therapies in the home or outpatient setting.
  • 7. Responsible for knowing and adhering to all home infusion pharmacy Policy and Procedures.
  • 8. Participate in inventory control, ordering, warehouse organization, packing/shipping, equipment maintenance and hazardous waste control.
  • 9. Responsible for emergency on-call duties on weekends and weeknights as required by center scheduling.
  • 10. Learn and utilize all computer programs necessary for the successful review, authorization, preparation, dispensing and management of home infusion pharmacy patients.
  • 11. Remain familiar with literature, references and resources for therapies and care of patients serviced by the home infusion center.
  • 12. Maintains the confidentiality of patients and their personal information at all times.

Qualifications

EDUCATION

Graduation from an accredited College of Pharmacy with a B.S. or PharmD degree.

EXPERIENCE

Completion of an ASHP-accredited PGY1 pharmacy residency or equivalent experience is desired, but not required.

LICENSURE

Connecticut Pharmacist Licensure required, active and without restrictions. BLS (Basic Life Support) certified or obtained within 6 months of hire.

SPECIAL SKILLS

This individual should demonstrate basic knowledge of pharmacology and therapeutics and understand pharmacy systems of drug distribution and formulary management. Excellent verbal and written communication skills and the ability to work with others are required. Must have expertise in patient management. Must pass all sterile compounding competencies as required by department.

PHYSICAL DEMAND

During the course of daily tasks, employee is required to walk, sit, stand and crouch. Must regularly lift/move up to 10 lbs., frequently life/move up to 25 lbs., and occasionally lift/move up to 50 lbs.
