Job Summary :

To assist in the delivery of primary health care and patient care management in a clinical and/or administrative capacity. Daily responsibilities of the position are based solely on the needs of the practice at any given time.

Job Description

Essential Responsibilities:

Data Systems Review
1. Assists the triage nurse by doing vital signs and escorting patients and families to designated rooms within departments. Does not make triage decision.

2. When a patient presents by ambulance may perform and document the following as directed and supervised by the RN.

  • A physical systems review including primary and secondary review under the direction and supervision of the professional nurse. As evidenced by random audits.
  • Begins interventions based on observation, interview of patient and/or significant other, physician diagnosis, lab findings, and other pertinent information, under the direction and supervision of the nurse.

3. Under the direction of the nurse reviews patient condition and vital signs then documents minimum of every two hours.

4. Performs and documents a history to include: chief complaint, past history, allergies with reactions, previous episodes and treatment, quality of pain or symptoms, aggravating or alleviating factors, specific location of complaint, weights for children under 12, blood pressures for children 3 years and older, treatment prior to arrival, vital signs, current medications, tetanus history, visual acuity for patient with eye injuries & last menstrual period.

Planning

1. Assists the nurse with plan of care based on observation and/or interview of the patient/significant other, physician's diagnosis, lab findings, and any other pertinent information.

2. Assists the nurse with documenting discharge/health teaching based on the patient/family/learning needs.

3. Participates in patient care conferences with the ER physician in order to plan and evaluate effective patient care.

4. Check and document on check off list, room assignments and crash carts with 100% compliance to ensure adequate supplies and equipment are available for patient care.

Implementation

1. Implements a plan of care based on systems review data as well as knowledge base as evidenced by documentation of written records, observation of performance and reporting of action.

2. Uses judgment to follow standards of care, consistent with systems review of patient's condition and physician's treatment plans under the direction and supervision of the nurse.

3. Assists with patient care in the critical care and general areas.

4. Performs routine equipment checks.

5. Identifies dysrhythmia and relays information to Nurses and Physicians.

6. Initiates treatments per protocol.

  • Cardiopulmonary Resuscitation Defibrillation
  • Intubation (ETT)
  • Administers emergency drugs per ACLS protocols.
  • Initiates oxygen therapy and performs oral and nasal pharyngeal suctioning.
  • Initiates intravenous lines and starts standard or premixed IV solutions as directed by nurse.
  • Inserts Foley Catheters.
  • Inserts Nasogastric Tubes.
  • Collects a specimen under the direction of the nurse. (Urine, blood secretions)
  • Performs local wound infiltration (no blocks).

7. May administer the following medications as directed by the nurse or physician:

  • All ACLS medication
  • Antacids
  • Aspirin
  • Albuterol
  • Acetaminophen
  • Ativan
  • Benadryl
  • Bicarbonate
  • D25W
  • D50W
  • Diazepam
  • Dilaudid
  • Diphteria/Tetanus Toxoid IM (Peds/Adults)
  • Duoneb
  • Fentanyl
  • Glucagon
  • Ibuprofen
  • Ketamine
  • Lasix
  • Levophed under supervision of Physician/Rn
  • Magnesium Sulfate
  • Midazolam
  • Morphine
  • Narcan
  • Nitroglycerine SL/paste
  • Racemic Epinephrine
  • RSI (Tactical medic only) under supervison of physician
  • Solumedrol
  • Toradol
  • Viscous Xylocaine PO
  • Zofran

8. Operates and checks equipment: cardiac monitors, defibrillators. Pacemakers, doppler - sphygmomanometer, dinamap - thermometers, rapid fluid infuser, MAST.

9. Transports patient via wheelchair or cart or treatment, diagnostic procedures and admissions to units.

10. Prepares patients for exams (e.g. disrobing, putting on gown, draping patient).

11. Documents and maintains records and charts according to department protocol, and reports significant changes in the patient conditions, medications given, and treatments administered to Nurse/Physician. Has documentation co-signed by the nurse.

Evaluation

1. Documents response to interventions as indicated by random chart audits: under the direction and supervision of the nurse. Including repeat vital signs if the patient was given medications which could alter vital signs, as well as their response to interventions.

2. Under Nurse Supervision evaluate the patient's condition and re-evaluate as needed to determine immediate/ongoing plan of care evidenced by documentation and observation.

Mandatory Licensure/Certifications:

Must be an Indiana State Emergency Medical Services Commission licensed EMT-Paramedic.

Experience:

Preferred experience is 1-2 years.
