SUMMARY

The Community Health Worker position provides family-centered, member-focused, relationship-based outreach and engagements services to Enhanced Care Management (ECM) eligible members and administrative support to Lead Care Coordinators. This position effectively utilizes Agency policies and procedures and makes appropriate independent judgments in the course of daily work.

ESSENTIAL DUTIES AND RESPONSIBILITIES
* Supports and promotes the mission of the Agency: Foothill Family empowers children and families to achieve success in relationships, school, and work through community-based services that advance growth and development.
* Engages eligible ECM members, performing outreach and engagement services.
* Drives or accompanies ECM members to office visits and in the most accessible setting, within managed care plan guidelines.
* Assists with health promotion and self-management training.
* Drives or arranges transportation for ECM members.
* Assists with linkage to social support.
* Distributes health promotion materials.
* Calls ECM eligible members to facilitate visits with the Lead Care Manager.
* Connects ECM members to other social services and support as they may need.
* Advocates on behalf of members with health care professionals.
* Utilizes motivational interviewing, trauma informed care, and harm reduction approaches.
* Monitors treatment adherence, including medication.
* Identifies gaps in and barriers to service, documenting in the outreach plan, and communicating these to the Lead Care Manager and participating in the development and implementation of responses.
* Prepares for and participates in individual and group supervision meetings.
* Behaves in an ethical, professional manner in support of Agency culture
* Identifies and raises for discussion, with management staff, potential workplace and morale issues, making recommendations for addressing them.
* Displays positive and attentive attitude at site meetings, as well as department, program and Agency staff meetings.
* Works effectively with the program and site coordinators and the department head, accepting direction and implementing policies and procedures reflective of this direction.
* Displays sensitivity to the service population's cultural and socioeconomics characteristics.
* Performs work in a safe manner at all times and ensures that other individuals do not put themselves or others at risk.
* Follows all policies, procedures and legal mandates including HIPAA, member confidentiality, releases of information, protected health information (PHI) and member files.
* Ensures performance and behaviors support the Agency's performance quality improvement (PQI) goals and objectives.
* Reports to work on time and maintains reliable and regular attendance.
* Performs other duties as assigned.

POSITION REQUIREMENTS

  • High School Diploma or equivalent, plus two years of outreach, collaboration and networking experience is required.
  • Excellent verbal and written communication skills; ability to conceptualize.
  • Lived experience navigating the mental health system for services or support programs or accessing other life opportunities such as education, employment and housing preferred.
  • Ability to document work in the member record, to draft other documents related to member work and to complete administrative reports with accuracy and timeliness.
  • Possess organizational skills to manage high activity level, provide needed services and attention to each member while maintaining all reporting and documentation requirements in a timely and accurate manner.
  • Excellent people and problem-solving skills; ability to work independently as well as a member of a team.
  • Ability to work with staff and members from diverse ethnic, cultural, and economic backgrounds, and to provide services in a non-judgmental manner.
  • Affinity for working with adolescents and young children.
  • Initiative, flexibility, creativity, and enthusiasm.
  • Ability to write routine reports and correspondence.
  • Valid CA Driver's License and maintains insurability on the Agency's auto liability policy (including a minimum of 2 years driving experience after receiving license) and maintains the California state required auto insurance liability limits.
  • Bilingual English/Spanish skills required.
