Position Summary

Healthy Oxford Hills, the Community Health department at Stephens Hospital, is seeking a dedicated and passionate Community Health Worker (CHW) to join our team. Community Health Workers (CHW) are both respected members of the communities they serve and key members of the health care team. CHWs positively impact the health of our patient populations through addressing the social, medical and community problems that may contribute to poor health. The CHW will assist patients to access health resources/services, support disease self-management and will assist in the coordination of culturally appropriate care and services for our patient populations.The CHW will represent the program in the community and will work with individuals, families, groups, and community partners to provide outreach, public education, and referrals as needed.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

1. Education: High school diploma or GED preferred. 2. License/Certifications: If driving required for role, Valid Driver's license, clean driving record, and personal vehicle for use required. 3. Experience: Two years of experience working in a social service or health care setting or equivalent educational setting preferred. 4. Additional Skills/Requirements Required: Must be a resident of the community being served with good knowledge of the resources of their community. 5. Additional Skills/Requirements Preferred: Basic computer skills preferred.
