Accountabilities
- Leads/develops the clinical program delivery for hospital at home across the system. Creates an environment of enthusiasm and excellence. Properly delegates responsibility, accountability and authority. Appropriately uses rewards and holds reports accountable. Supports and communicates Prisma Health mission, vision, and values, throughout the organization. Develops departmental strategic and operational plans to achieve the mission and vision of the organization in collaboration and by direction of the Executive Director. Establishes policies and protocols for clinical care delivery in the home in collaboration with Home Recovery Care medical director. Develops and maintains the department plan that is integrated in the Prisma Health hospice service line. Demonstrates job competency. Provides workable solutions utilizing Lean principles, technique processes and procedures.
- Develops the growth and clinical strategies, Analyzes data/information in relation to making decisions. Develops thorough operational plans that achieve business objectives/goals. Plans and coordinates work with other work groups/departments. Brings to bear the appropriate knowledge, information, and expertise in making decisions. Considers strategic issues in making decisions. Bases decisions on sound logic rationale. Advances problems towards resolution despite ambiguity or uncertainty. - Developing People: Hires, retains, and manages a well-qualified and diverse staff. Ensures appropriate staffing levels to achieve outcomes. Evaluates, coaches, and mentors others through feedback processes. Provides in-service training and continuing education of all persons in the department. Assures that all certifications and competencies required and recommends sufficient number of staff to support and carry out the provision of services. Develops and ensures completion of Introductory/Orientation Period Checklists and competencies. Meets with staff to discuss what is important to them, their goals and management expectations.
- Standards & Accountability: Ensures all regulatory requirements are met. Ensures staff understands the goals/tasks they are accountable for achieving. Sets performance improvement priorities and ensures that important processes and activities are measured, assessed, and improved systematically. Identifies and implements appropriate patient safety strategies relative to the department. Ensure survey readiness for Joint Commission, DHEC, CMS, and other regulatory agencies. Demonstrates and ensures employees' compliance with consistent behavior in accord with the expectations for excellent customer service to patients, families, physicians, visitors, employees and others. Responsible for patient safety, quality, and satisfaction delivery.
- Communicating & Managing Relationships: Shares information and feedback with direct reports and co-workers appropriately. Expresses concerns and makes suggestions in a manner consistent with spirit of cooperation and collaboration. Demonstrates positive verbal and non-verbal communication skills that enhance the work environment. Provides leadership and direction that promotes an efficient and effective staff and fosters open communication. Coordinates processes with those of other departments and health care related organizations. Develops a culture that emphasizes cooperation and communication between individual staff members, other departments, and medical staff. Develops policies and procedures in collaboration with associated departments. Participates in interdepartmental committees to foster communication and integration. Coordinates for Provision of Services not available at Prisma Health, Oconee Memorial Hospital. Develops relationships with higher management. Cultivates networks of people across a variety of functions and departments within the organization. Actively engaged in community activities and provides public speaking and presentations for hospice services on an ongoing basis.
- Collaborates directly with physician leaders for program growth/development; with hospital CEOs and CNOs for program delivery, Care Management leadership in all markets for transitions of care, and with ED leadership across the system for transition of patients from ED to home.
- As needed, provide direct clinical care (face to face or virtually) to support the clinical care delivery services essential for acute care services in the patient's home.
Supervisory/Management Responsibilities Job has direct and/or indirect supervision of employees that may include final budget authority, hire/termination/disciplinary authority, performance appraisal responsibility. Job will be considered a member of the management staff with direct reports. Minimum Requirements
Bachelor's in Nursing required. MSN preferred. 3 years of acute care nursing experience required with ICU experience preferred. 10+ years of clinical experience caring for complex patients preferred. Management experience preferred. SC Advanced Practice Practitioner (NP or PA) preferred.
Required Certifications/Registration/Licensure RN - REGISTERED NURSE licensure to practice in South Carolina. SC DRIVERS LICENSE - LSCD Work Shift Day (United States of America)
